Keloids caused by acne scars?
I have developed keloids around my jawline due to severe acne in the past.
I am currently undergoing treatment with corticosteroids, which has reduced their size somewhat, but they are still quite large.
Are there any better treatment options available? Or are there cosmetic procedures that could be effective? Thank you!

Unfortunately, there is currently no perfect treatment option.
Local steroid injections may be the most effective method.

Keloids, which are raised scars that develop from an overproduction of collagen during the healing process, can be particularly frustrating for those who have experienced severe acne. The chin area is a common site for keloid formation, especially after acne lesions have healed. Your current treatment with corticosteroids is a common first-line approach, but there are several other effective treatments and strategies that you might consider.
1. Corticosteroid Injections: As you mentioned, you are already using corticosteroids, which can help flatten keloids over time. These injections are typically administered every few weeks and can reduce inflammation and collagen production in the keloid tissue. While you have seen some improvement, it may take multiple sessions to achieve the desired results.
2. Silicone Gel Sheets or Silicone Gel: Silicone therapy is a well-established treatment for keloids. Silicone sheets or gels can be applied to the keloid and worn for several hours a day. This method helps to hydrate the scar tissue and can reduce its size and redness over time. It is non-invasive and can be used in conjunction with other treatments.
3. Laser Therapy: Laser treatments, such as pulsed dye laser or fractional laser, can be effective in reducing the size and appearance of keloids. These lasers work by targeting the blood vessels in the keloid, reducing redness and promoting a more even skin texture. Multiple sessions may be required, and it is essential to consult with a dermatologist who specializes in laser treatments.
4. Cryotherapy: This involves freezing the keloid with liquid nitrogen, which can help reduce its size. Cryotherapy is often used in conjunction with other treatments, such as corticosteroid injections, for better results.
5. Surgery: In some cases, surgical excision of the keloid may be considered. However, it is important to note that surgical removal can sometimes lead to the keloid returning, potentially larger than before. Therefore, surgery is often combined with other treatments, such as corticosteroid injections or radiation therapy, to minimize the risk of recurrence.
6. Radiation Therapy: This is a less common but effective treatment for keloids, particularly after surgical excision. Low-dose radiation can help prevent the keloid from returning after it has been removed.
7. Topical Treatments: Some topical treatments, such as 5-fluorouracil (5-FU) or imiquimod, have been used to treat keloids. These medications can help reduce the size of the keloid and improve its appearance.
8. Microneedling: This technique involves using fine needles to create micro-injuries in the skin, which can stimulate collagen production and improve the texture of the keloid. It is often used in combination with other treatments for enhanced results.
9. Dermatological Consultation: It is crucial to work closely with a dermatologist or a specialist in scar management. They can assess your specific situation and recommend a tailored treatment plan that may include a combination of the above options.
In addition to these treatments, maintaining a healthy skincare routine and protecting the area from sun exposure can also help improve the overall appearance of the skin. Remember that keloid treatment can take time, and patience is essential. It is advisable to discuss all available options with your healthcare provider to determine the best course of action for your specific case.
