Testicular pain and a hard lump?
Hello, I have a question.
Recently, I've been experiencing pain in my right testicle that feels heavy and aching.
When I touch it, it seems like about one-third of the testicle has hardened.
Additionally, I've noticed blood in my semen during ejaculation.
Could this be a symptom of cancer, and would I need surgical treatment?
Another issue for your reference is that I have been experiencing semen leakage from my urethra during bowel movements, and the amount is significant.
This symptom has been present for two years.
Is there a connection between this and the previous issue? Am I in serious danger?

Based on your description, it is recommended that you visit a urology clinic for a physical examination and relevant tests to differentiate between the possibilities of inflammation or a tumor.
As for the simultaneous expulsion of semen during bowel movements, it may not necessarily be related to the aforementioned issue and is not uncommon among young males.
This phenomenon is primarily due to the fullness of the seminal vesicles and generally does not indicate any significant problem.

Testicular pain and hardness can be concerning symptoms that warrant immediate medical attention. Based on your description, it sounds like you are experiencing significant discomfort in your right testicle, along with a hardening sensation and the presence of blood in your semen (hematospermia). These symptoms can be indicative of several conditions, some of which may require urgent evaluation and treatment.
First, let's address the potential causes of your symptoms. Testicular pain can arise from various issues, including:
1. Epididymitis: This is an inflammation of the epididymis, often caused by infections, including sexually transmitted infections (STIs). Symptoms may include pain, swelling, and sometimes fever.
2. Orchitis: This is inflammation of the testicle, which can also be due to infections, including viral infections like mumps.
3. Testicular torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. It typically presents with sudden, severe pain and requires immediate surgical intervention.
4. Inguinal hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles, which can cause discomfort in the groin and testicular area.
5. Tumors: Testicular cancer can present with a hardening of the testicle, pain, and sometimes changes in the semen. While it is less common, it is a serious condition that needs to be ruled out.
The presence of blood in your semen is particularly concerning and can be associated with several conditions, including infections, trauma, or even tumors. The fact that you also mentioned experiencing semen leakage during bowel movements raises additional concerns. This could suggest a possible connection to a prostate issue or other underlying conditions affecting the reproductive system.
Given the combination of symptoms you are experiencing, it is crucial to seek medical attention as soon as possible. A healthcare provider will likely perform a physical examination, order imaging studies (such as an ultrasound), and possibly conduct blood tests to determine the underlying cause of your symptoms. Early diagnosis and treatment are vital, especially if there is a risk of testicular torsion or cancer.
Regarding your concern about whether you are "close to death," it is important to remember that while some conditions can be serious, many can be effectively treated when caught early. Avoid jumping to conclusions about the severity of your situation without a proper medical evaluation.
In summary, your symptoms of testicular pain, hardness, and blood in your semen are concerning and should be evaluated by a healthcare professional promptly. Do not delay in seeking care, as timely intervention can make a significant difference in outcomes for many testicular conditions.
