Abnormal genital bleeding?
Hello Dr.
Zheng,
My menstrual period ended around May 16 or 17.
I had sexual intercourse with my boyfriend on May 23.
During the process, I experienced significant pain, but my boyfriend continued to apply pressure.
I was worried that his penis might have caused some internal injury because the same motion was forcefully repeated for a long time.
Since it was my first time, we didn't really know what to expect and thought that some pain was normal.
Afterward, I noticed slight bleeding from my genital area, which has continued until today (May 28), along with persistent pain.
Yesterday, I started to feel that something was not right and became very anxious about the possibility of future infertility, so I mustered the courage to visit a private gynecologist.
However, I didn't tell the doctor that my symptoms were related to having sexual intercourse.
The doctor performed an ultrasound and said everything looked fine, with no issues.
I also mentioned that I experienced pain while urinating and some slight bleeding, but the doctor reassured me that it was nothing to worry about and prescribed anti-inflammatory medication.
I just want to ask...
is everything really okay? I still feel pain, especially when urinating! The anti-inflammatory medication has helped a lot, and I don't feel as much pain during normal times, but there is still something that feels off.
I still experience pain while urinating, along with some abdominal discomfort, and I have a constant urge to urinate.
My frequency of urination has increased, but the amount is very little, just a bit.
Thank you, doctor! This is my first experience with such a situation, and it's quite scary.
I hope to receive some answers.

Hello: Based on your description, you are experiencing pain from the hymenal tear after your first sexual intercourse and have developed cystitis (honeymoon cystitis).
You have seen some improvement after taking anti-inflammatory medication, but you are not completely better.
This may be due to insufficient duration of medication.
It is recommended that you visit a hospital to have your urine tested to assess the level of inflammation, continue using antibiotics, and increase your water intake and bathroom visits.
Sincerely, Dr.
Cheng Jin-He, Department of Obstetrics and Gynecology, Hsinchu Hospital.

It's understandable to feel concerned after experiencing abnormal bleeding and discomfort following your first sexual encounter. Based on your description, there are several factors to consider regarding your symptoms.
Firstly, it's important to note that experiencing pain during your first sexual experience is not uncommon. Many individuals may feel discomfort or pain due to anxiety, lack of arousal, or insufficient lubrication. The fact that you mentioned your boyfriend was applying significant force could have contributed to the discomfort and potential trauma to the vaginal area. The vagina is a sensitive structure, and any vigorous activity, especially during the first experience, can lead to minor injuries or abrasions, which may result in bleeding.
The bleeding you are experiencing could be attributed to several causes:
1. Vaginal Trauma: As mentioned, if there was vigorous activity, it's possible that minor tears or abrasions occurred in the vaginal lining, leading to bleeding. This type of bleeding is usually light and may resolve on its own.
2. Hymenal Tears: If you are a virgin, the hymen (a thin membrane that partially covers the vaginal opening) may have been stretched or torn during the encounter, which can also cause bleeding. However, not all individuals bleed during their first sexual experience, and the hymen can vary significantly in its structure.
3. Infection or Inflammation: The pain during urination and the presence of blood in your urine could indicate a urinary tract infection (UTI) or inflammation of the urinary tract. UTIs can occur after sexual activity, especially if proper hygiene is not maintained. Symptoms of a UTI include increased urgency to urinate, pain during urination, and sometimes blood in the urine.
4. Hormonal Factors: Since you mentioned that your menstrual period ended shortly before the sexual encounter, hormonal fluctuations could also play a role in abnormal bleeding. Sometimes, the body may react to stress or physical trauma with unexpected bleeding.
Given that you have already seen a gynecologist and received reassurance that everything appears normal on the ultrasound, it's a positive sign. However, the persistent pain during urination and the ongoing bleeding are concerning. It's crucial to monitor your symptoms closely. If the bleeding continues or worsens, or if the pain becomes severe, you should seek further medical evaluation.
In the meantime, here are some recommendations:
- Hydration: Drink plenty of water to help flush out your urinary system, which may alleviate some discomfort if a UTI is present.
- Hygiene: Maintain good genital hygiene, especially after sexual activity, to reduce the risk of infections.
- Avoid Irritants: Avoid using scented soaps or products in the genital area that could cause irritation.
- Rest: Allow your body time to heal. If you experience pain, consider taking over-the-counter pain relief as advised by your healthcare provider.
Lastly, communication with your partner is essential. Discussing comfort levels and boundaries can help ensure that future experiences are more enjoyable and less painful. If you continue to feel anxious or fearful about sexual activity, consider speaking with a healthcare professional or a counselor who specializes in sexual health to address these concerns.
In summary, while some discomfort and light bleeding can be normal after a first sexual experience, persistent symptoms warrant further investigation. It's always better to err on the side of caution and seek medical advice if you're unsure about your symptoms.
