Uterine fibroids
Since last September, my menstrual period has not come, and I only found out in February that I have a 4.8 cm fibroid.
I have received two injections to induce menstruation and have taken progesterone, but my period still hasn't returned.
Is this due to the fibroid issue, or could it be related to infertility?

Dr. Wei Fumao reply Obstetrics and Gynecology
Pregnancy must be ruled out first.
If there is no pregnancy, menstruation will still occur; it is just a matter of time.

More Info
Uterine fibroids, also known as leiomyomas, are non-cancerous growths that develop in the uterus. They are quite common, especially among women of reproductive age, and can vary in size from very small to large masses that can distort the shape of the uterus. Understanding the impact of fibroids on the menstrual cycle and fertility is crucial for women experiencing symptoms or who are trying to conceive.
Impact on Menstrual Cycle
Fibroids can significantly affect a woman's menstrual cycle. Many women with fibroids experience heavy menstrual bleeding (menorrhagia), prolonged periods, or irregular bleeding. This is primarily due to the fibroids' ability to interfere with the normal contraction of the uterine muscles, leading to excessive blood flow. In some cases, fibroids can also cause pain during menstruation (dysmenorrhea) or pelvic pain, which can further complicate a woman's menstrual health.
In your case, the absence of menstruation (amenorrhea) could be related to the presence of the fibroid. The size of the fibroid (4.8 cm) may be significant enough to disrupt the normal hormonal signals that regulate the menstrual cycle. Additionally, if you have been receiving hormonal treatments such as progestin (like the luteal hormone you mentioned), these can also affect your menstrual cycle, potentially leading to missed periods.
Impact on Fertility
When it comes to fertility, the relationship between fibroids and the ability to conceive is complex. Fibroids can impact fertility in several ways:
1. Location: The location of the fibroid is critical. Submucosal fibroids, which grow just beneath the lining of the uterus, are most likely to interfere with implantation of an embryo. Intramural fibroids, which grow within the uterine wall, can also affect the shape of the uterine cavity and may impact fertility.
2. Size: Larger fibroids can distort the uterine cavity, making it more difficult for an embryo to implant successfully. They can also interfere with the blood supply to the uterus, which is essential for a healthy pregnancy.
3. Hormonal Environment: Fibroids can alter the hormonal environment of the uterus, potentially affecting ovulation and the menstrual cycle, which can indirectly impact fertility.
4. Associated Conditions: Women with fibroids may also have other conditions that affect fertility, such as endometriosis or pelvic inflammatory disease.
Next Steps
Given your situation, where you have been diagnosed with a fibroid and have experienced amenorrhea despite treatment, it is essential to consult with a healthcare provider who specializes in reproductive health. They may recommend further evaluation, such as an ultrasound or MRI, to assess the fibroid's size and location and determine if it is impacting your fertility.
If you are considering pregnancy, your healthcare provider may discuss options such as:
- Monitoring: If the fibroid is not causing significant symptoms or complications, monitoring it may be a reasonable approach.
- Surgical Options: In some cases, surgical removal of the fibroid (myomectomy) may be recommended, especially if it is believed to be affecting your fertility.
- Fertility Treatments: Depending on your overall reproductive health, fertility treatments such as in vitro fertilization (IVF) may be considered.
Conclusion
In summary, uterine fibroids can have a significant impact on both the menstrual cycle and fertility. The absence of menstruation in your case could be related to the fibroid, hormonal treatments, or other factors. It is crucial to work closely with your healthcare provider to understand your specific situation and explore the best options for managing your fibroids and optimizing your chances of conception.
