Regarding issues related to the glans penis?

Dear Doctor,
I have had a slightly elongated foreskin since childhood, and it has a semi-open appearance.
However, at around one month of age, I noticed several red spots on the glans, and I also experience a slight stinging sensation at the urethral opening.
To this day, I can tell when these red spots reappear because I feel a slight sensation at the urethral opening.
I previously consulted a urologist who diagnosed me with balanitis due to phimosis and advised me to use an anti-inflammatory ointment.
However, after a month, I still occasionally experience this issue.
I really don’t know what this disease is.
Could you please help me understand the cause of my condition? Thank you...
I am quite troubled...

Generally, if there is no redness, swelling, or broken skin, it is less concerning.
However, it is best to return for a follow-up examination.

Phimosis and balanitis are conditions that can significantly affect male genital health, and understanding them is crucial for effective management. Phimosis refers to the inability to retract the foreskin over the glans (the head of the penis), which can be a congenital condition or develop over time due to scarring or inflammation. Balanitis, on the other hand, is the inflammation of the glans, often accompanied by symptoms such as redness, swelling, and pain. It can be caused by infections, irritants, or underlying skin conditions.

In your case, it seems that you have experienced symptoms consistent with both phimosis and balanitis. The presence of red spots on the glans and a stinging sensation at the urinary opening could indicate an inflammatory response, possibly due to balanitis. This condition can arise from various factors, including poor hygiene, irritants (such as soaps or lotions), infections (like yeast or bacterial infections), or even sexually transmitted infections.

The fact that you have been diagnosed with "包皮龜頭炎" (balanitis) by a urologist suggests that your symptoms are indeed related to inflammation of the glans, likely exacerbated by the phimosis. The recommendation to use anti-inflammatory ointments is a common approach to manage mild cases of balanitis. However, if you continue to experience recurrent symptoms despite treatment, it may indicate that the underlying issue, such as phimosis, needs to be addressed more directly.

Phimosis can lead to difficulties in hygiene, increasing the risk of infections like balanitis. In some cases, if phimosis is severe and causes recurrent infections or discomfort, surgical intervention, such as circumcision or preputioplasty, may be considered. These procedures can help alleviate the symptoms by allowing for better hygiene and reducing the risk of inflammation.

It's essential to maintain good hygiene practices, especially if you have phimosis. Gently cleaning the area with mild soap and water can help prevent the buildup of smegma, which can contribute to irritation and infection. If you notice any new or worsening symptoms, such as increased pain, discharge, or persistent redness, it is crucial to follow up with your healthcare provider for further evaluation.

In summary, your symptoms suggest a combination of phimosis and balanitis, which can be managed with proper hygiene and topical treatments. However, if your symptoms persist, further evaluation and possibly surgical options may be necessary to ensure your long-term genital health. Always consult with a qualified healthcare professional for personalized advice and treatment options tailored to your specific situation.
