Itching of the glans and foreskin?

Hello Doctor:
1.
Recently, I have noticed itching on the glans and inside the foreskin.
The itchy areas appear as small red bumps, approximately 1mm in size and about 0.5mm in height.
What could be the cause of this? Is it eczema? What medications are available for treatment? Can I apply medication on my own?
2.
During intercourse with my girlfriend, we always use condoms.
After several instances of intercourse, I experience skin breakdown and pain on the foreskin, which occurs only after ejaculation.
Could this be an allergic reaction to the condom or is there another reason? (The foreskin can retract normally.) Thank you.

Dr. Jiang Zheen reply Dermatology

It is best to have a dermatologist examine symptoms of skin diseases, especially if there is broken skin or pain, as this may be due to insufficient lubrication.
Wishing you safety and well-being.

Understanding itchy red bumps on the glans and foreskin can be concerning, and it's important to address these symptoms with care. Based on your description, the red, itchy bumps on the glans and foreskin could be caused by several factors, including dermatitis, infections, or allergic reactions. Here’s a breakdown of potential causes and treatment options.

1. Possible Causes:
- Contact Dermatitis: This is a common cause of itchy, red bumps and can occur due to irritation from soaps, lotions, or even the latex in condoms. If you've recently changed any products that come into contact with your genital area, this could be a factor.

- Eczema or Psoriasis: These skin conditions can manifest as red, itchy patches and may affect the genital area. They can be exacerbated by stress, weather changes, or irritants.

- Fungal Infections: Conditions like candidiasis (yeast infection) can cause red, itchy bumps. This is more common in warm, moist environments.

- Bacterial Infections: Certain bacterial infections can also lead to similar symptoms.

- Sexually Transmitted Infections (STIs): Some STIs can present with red bumps or lesions. If you have concerns about STIs, it’s crucial to get tested.

2. Treatment Options:
- Topical Steroids: If the bumps are due to dermatitis or eczema, a mild topical steroid cream (like hydrocortisone) may help reduce inflammation and itching. However, it’s essential to use these under the guidance of a healthcare provider, especially in sensitive areas.

- Antifungal Creams: If a fungal infection is suspected, over-the-counter antifungal creams may be effective.

- Avoid Irritants: Identify and avoid any products that may be causing irritation, including soaps, lotions, or laundry detergents.

- Moisturizers: Keeping the area moisturized can help alleviate dryness and irritation, especially if eczema is a concern.

3. Self-Treatment Considerations:
- While some over-the-counter treatments can be effective, it’s advisable to consult a healthcare professional before starting any treatment. They can provide a proper diagnosis and recommend the most suitable treatment based on your specific condition.

- If symptoms persist or worsen, or if you notice any additional symptoms (such as discharge, fever, or increased pain), seek medical attention promptly.

4. Regarding Pain During Intercourse:
- The pain you experience after ejaculation, particularly if it occurs consistently, could be due to friction or irritation from the condom. It’s also possible that you may have a sensitivity or allergy to latex or other materials used in condoms. Consider trying non-latex condoms to see if that alleviates the issue.

- If the pain continues, or if you notice any lesions or persistent irritation, it’s crucial to consult a healthcare provider. They may perform a physical examination and possibly recommend allergy testing or other evaluations to determine the underlying cause.

In summary, while the symptoms you describe could be due to various factors, it’s essential to approach treatment carefully. Consult with a healthcare professional for an accurate diagnosis and tailored treatment plan. They can help ensure that any underlying issues are addressed and provide guidance on safe and effective self-care measures.
