Ankle sprain issues..?

A few days ago, my sister twisted her ankle after missing a step on the stairs (it has been swollen for 4.5 days now).
On the day of the injury, she went to a traditional Chinese medicine clinic, where they treated her and wrapped her ankle.
However, after several days, there was no improvement, and the swelling increased.
The clinic recommended that we get an X-ray.
The doctor at that hospital said there were no fractures, just internal bruising, and they performed a blood draw.
After returning home, there was still no improvement, and the site of the blood draw became bruised and swollen (on the injured leg).
Later, we went to another hospital, where the doctor said there was a hairline fracture in her ankle.
However, he reassured us that it would heal naturally and advised her to apply ice.
Despite icing, her ankle has not improved; in addition to the swelling, blisters have developed.
Therefore, I would like to ask the doctor how we should proceed with her treatment.
Thank you.

Dear Sir/Madam,
The most crucial time for managing an ankle sprain is within the first seventy-two hours post-injury.
Active treatment should include ice application, anti-inflammatory medications, elevation of the foot, rest, and protection.
Since your sister missed this critical window and is experiencing swelling and blisters due to the bandaging, it will take a considerable amount of time for the skin around the ankle to heal.
Currently, the only options available for management are anti-inflammatory medications and protective measures to reduce swelling and pain, which may take approximately 6 to 8 weeks.
Sincerely,
Dr.

Managing ankle sprains effectively is crucial for a successful recovery and to prevent long-term complications. Ankle sprains are common injuries that occur when the ligaments supporting the ankle are stretched or torn, often due to a sudden twist or impact. Given your sister's situation, where she has experienced significant swelling and discomfort following her injury, it is essential to follow a structured approach to treatment.

Immediate Care
1. R.I.C.E. Method: The first line of treatment for an ankle sprain is the R.I.C.E. method, which stands for Rest, Ice, Compression, and Elevation.

- Rest: Avoid putting weight on the injured ankle. This may require the use of crutches or a brace to stabilize the joint.

- Ice: Apply ice packs to the affected area for 15-20 minutes every 1-2 hours. This helps reduce swelling and numbs the pain. Ensure that ice is not applied directly to the skin to prevent frostbite.

- Compression: Use an elastic bandage or compression wrap to help minimize swelling. Be careful not to wrap it too tightly, as this can impede circulation.

- Elevation: Keep the injured ankle elevated above the level of the heart to help reduce swelling.

2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation. However, it is essential to follow the recommended dosages and consult a healthcare provider if there are any concerns, especially considering your sister's medical history.

Medical Evaluation
Since your sister's condition has not improved and she has developed blisters, it is crucial to seek further medical evaluation. The presence of blisters may indicate a more severe injury or complications such as skin irritation due to swelling or improper care. Here are some steps to consider:
1. Follow-Up with a Healthcare Provider: If the swelling and pain persist or worsen, it is advisable to consult an orthopedic specialist or a sports medicine physician. They may recommend further imaging studies, such as an MRI, to assess the extent of the injury and rule out any fractures or significant ligament tears.

2. Physical Therapy: Once the acute phase of the injury has passed, physical therapy can be beneficial. A physical therapist can design a rehabilitation program that focuses on restoring strength, flexibility, and stability to the ankle. This may include exercises to improve range of motion and proprioception, which is crucial for preventing future sprains.

Recovery Tips
1. Gradual Return to Activity: Once your sister begins to feel better, she should gradually return to her normal activities. It is important to listen to her body and avoid pushing through pain, as this can lead to re-injury.

2. Strengthening Exercises: Incorporating ankle strengthening exercises into her routine can help prevent future sprains. These may include resistance band exercises, calf raises, and balance training.

3. Footwear: Ensure that your sister wears supportive footwear that provides adequate stability for her ankles, especially when returning to physical activities.

4. Hydration and Nutrition: Staying hydrated and maintaining a balanced diet can support the healing process. Nutrients such as protein, vitamin C, and zinc are essential for tissue repair.

Conclusion
In summary, managing an ankle sprain involves immediate care through the R.I.C.E. method, proper medical evaluation, and a structured rehabilitation program. Given the complications your sister is experiencing, it is vital to seek further medical advice to ensure appropriate treatment and recovery. With the right approach, she can return to her normal activities and minimize the risk of future injuries.
