Breast tenderness and missed menstruation?

Hello Doctor,
I have irregular menstruation; in the past two years, I have only had my period five times.
It has been six months since my last period, and the last time I went eight months without menstruating, I received an injection to induce my period.
I have seen many obstetricians and gynecologists, but they did not conduct thorough examinations and just prescribed ovulation medication, stating that this condition would not improve without treatment for a year and a half.
This sounds very discouraging.
Some doctors say I have polycystic ovary syndrome (PCOS), while others say I do not.
Some prescribed medication without examination and advised me to lose weight.
I have gained about 20 kilograms in the past two years.
I have noticed that my neck has become thicker, my abdomen has enlarged significantly, and I have developed dark, coarse hair on my legs and abdomen.
For the past few months, my breasts have been consistently swollen and painful, and I also experience discomfort in my abdominal and pelvic areas.
Could you please advise me on which specialist I should see? Additionally, could you recommend a doctor in Taichung City? Thank you for your kind response; I appreciate it!

Hello: You should go to a larger hospital's obstetrics and gynecology department for hormone testing.

More Info

It sounds like you're experiencing a complex set of symptoms that could be related to hormonal imbalances, possibly linked to conditions such as polycystic ovary syndrome (PCOS) or other endocrine disorders. Irregular periods, breast pain, and changes in body hair and weight can all be interconnected and may require a comprehensive evaluation to determine the underlying cause.

Understanding Your Symptoms
1. Irregular Periods: Irregular menstrual cycles can be caused by a variety of factors, including hormonal imbalances, stress, significant weight changes, and underlying health conditions like PCOS. In PCOS, the ovaries may develop numerous small cysts, leading to irregular ovulation and menstrual cycles.

2. Breast Pain: Breast pain, particularly if it is cyclical (occurring in relation to your menstrual cycle), can be influenced by hormonal changes. However, persistent breast pain that does not correlate with your cycle may warrant further investigation to rule out other causes.

3. Weight Gain and Body Changes: The weight gain you've described, along with changes in body hair and fat distribution, can be indicative of hormonal issues, particularly related to insulin resistance, which is common in PCOS.
4. Physical Symptoms: The thickening of the neck, abdominal distension, and changes in hair growth can be signs of metabolic syndrome or other endocrine disorders. These symptoms should not be overlooked, as they can significantly impact your overall health.

Seeking the Right Specialist
Given the complexity of your symptoms, it would be advisable to consult with a specialist who can provide a comprehensive evaluation. Here are some recommendations:
1. Endocrinologist: An endocrinologist specializes in hormonal disorders and can perform detailed tests to assess your hormone levels, including insulin, testosterone, and others that may be contributing to your symptoms.

2. Reproductive Endocrinologist: If you are considering fertility options or have specific concerns related to your menstrual cycle and reproductive health, a reproductive endocrinologist would be beneficial. They can provide targeted treatment for conditions like PCOS.

3. Gynecologist: A gynecologist can also be a good starting point, especially one who specializes in hormonal disorders. They can perform necessary examinations and tests to rule out other potential issues.

Recommendations for Next Steps
- Comprehensive Testing: Request a full hormonal panel, including tests for thyroid function, insulin levels, and androgens. This can help clarify whether you have PCOS or another hormonal imbalance.

- Lifestyle Changes: While weight loss can be challenging, it is often recommended for managing symptoms of PCOS and improving insulin sensitivity. A balanced diet and regular exercise can be beneficial.

- Seek a Second Opinion: If you feel that your concerns are not being adequately addressed, consider seeking a second opinion from another specialist. It’s important to find a healthcare provider who listens to your concerns and takes them seriously.

Finding a Specialist in Taichung
For specific recommendations in Taichung, I would suggest looking for hospitals or clinics that have a dedicated women's health or endocrinology department. You can also check online reviews or ask for referrals from friends or family who may have had similar experiences.
In conclusion, your symptoms warrant a thorough evaluation by a specialist who can provide a tailored approach to your health concerns. Don't hesitate to advocate for yourself and seek the care you need to address these issues comprehensively.
