Penile correction

What are the surgical methods for penile correction? How is the correction performed? Will there be any residual effects? Thank you.

Jr., 20~29 year old female. Ask Date: 2001/04/17

Dr. Pan Zhenqin reply Urology

I would like to discuss the corrective surgery for penile curvature.
This procedure is performed under regional anesthesia, where the tunica albuginea of the penile corpora is simply sutured in the opposite direction of the curvature to straighten the penis.
The surgery is generally low-risk; however, there may be a slight reduction in penile length postoperatively.

More Info

Penile correction surgery, often referred to as penile lengthening or penile curvature correction surgery, is a procedure designed to address various penile deformities, including curvature (often seen in conditions like Peyronie's disease), as well as to enhance penile length or girth. Understanding the methods, outcomes, and risks associated with these surgeries is crucial for anyone considering such a procedure.

Methods of Penile Correction Surgery
1. Penile Lengthening Surgery: This procedure typically involves cutting the suspensory ligament that attaches the penis to the pubic bone. By severing this ligament, the penis can be pulled forward, which may result in a modest increase in length. Some surgeons may also perform liposuction in the pubic area to enhance the appearance of length.

2. Penile Girth Enhancement: This can be achieved through various methods, including fat grafting (lipofilling) or the use of dermal fillers. In fat grafting, fat is harvested from another part of the body and injected into the penis to increase girth. Fillers can also be injected to achieve a similar effect.

3. Curvature Correction Surgery: For conditions like Peyronie's disease, surgical options include plication techniques, where sutures are placed on the side of the penis opposite the curve to straighten it, or grafting techniques, where tissue is added to the shorter side of the penis to correct the curvature.

4. Penile Prosthesis: In cases where there is significant erectile dysfunction or penile deformity, a penile prosthesis may be implanted. This device can help achieve an erection and may also help in correcting curvature.

Outcomes of Surgery
The outcomes of penile correction surgery can vary widely depending on the specific procedure performed, the skill of the surgeon, and the individual patient's anatomy and health. Many patients report satisfaction with the results, particularly in terms of improved self-esteem and sexual function. However, the degree of improvement can be modest, and not all patients achieve the desired results.

Risks and Complications
As with any surgical procedure, penile correction surgery carries risks. Some potential complications include:
1. Infection: Any surgical procedure carries a risk of infection, which can complicate recovery and lead to further issues.

2. Scarring: Surgical incisions can lead to scarring, which may affect the appearance and function of the penis.

3. Erectile Dysfunction: There is a risk of developing erectile dysfunction following surgery, particularly if nerves or blood vessels are damaged during the procedure.

4. Changes in Sensation: Some patients may experience changes in penile sensation, which can be temporary or permanent.

5. Dissatisfaction with Results: Not all patients achieve the desired outcomes, and some may be dissatisfied with the results, leading to psychological distress.

6. Deformity: In some cases, the surgery may not fully correct the curvature or may lead to new deformities.

Conclusion
Before considering penile correction surgery, it is essential to consult with a qualified urologist or surgeon who specializes in this field. They can provide a thorough evaluation, discuss the potential benefits and risks, and help set realistic expectations regarding the outcomes. It is also advisable to seek a second opinion and to consider non-surgical options or therapies that may be available, particularly for conditions like Peyronie's disease. Ultimately, the decision to undergo surgery should be made after careful consideration of all factors involved, including the potential for complications and the psychological impact of the surgery.
