What could be the causes of discomfort in the right upper abdomen?

Recently, I often feel a strange sensation in my upper right abdomen (just below the chest, within the rib cage).
It doesn't feel like pain or nausea; it's more like a feeling of pressure.
I'm not very certain about it because the sensation is mild, and if I'm distracted by other things, I don't notice it.
I'm unsure what this area is, and since I am a carrier of hepatitis B, I wonder if the area I'm referring to is related to the liver.
Other than that, everything else feels normal, and it doesn't seem like any other disease, but I still find it concerning.
What could this situation indicate? Which specialty should I consult, and what kind of tests should I undergo? Thank you for your response, doctor.

Dr. Cai Zonglong reply Internal Medicine

Hello, your symptoms are similar to those experienced by many carriers of Hepatitis B.
Generally, it could be due to fatty liver or other causes that may be affecting the nerves.
I recommend that you visit a hepatobiliary department at a hospital for an ultrasound to rule out any potential lesions.
Sincerely, Dr.

Understanding discomfort in the upper right abdomen can be quite perplexing, especially when the sensation does not clearly align with typical pain or nausea. The area you are describing, located beneath the rib cage on the right side, is indeed where the liver is situated, along with other organs such as the gallbladder, parts of the small intestine, and the right kidney. Given your history as a carrier of hepatitis B, it is prudent to consider liver-related issues as a potential cause of your discomfort.

The sensation you describe as a feeling of pressure or discomfort could be attributed to several factors. Here are some possibilities:
1. Liver Conditions: Since you are a carrier of hepatitis B, it is essential to monitor your liver health. Chronic hepatitis can lead to liver inflammation (hepatitis), fibrosis, or even cirrhosis over time. These conditions can sometimes manifest as discomfort in the upper right abdomen. If your liver is enlarged (hepatomegaly), it may press against surrounding structures, causing discomfort.

2. Gallbladder Issues: The gallbladder is located just beneath the liver and can also be a source of discomfort. Conditions such as gallstones or cholecystitis (inflammation of the gallbladder) can lead to sensations of pressure or discomfort, especially after eating fatty meals.

3. Musculoskeletal Causes: Sometimes, discomfort in the upper right abdomen may not be related to internal organs at all. It could stem from muscle strain or tension in the abdominal wall or intercostal muscles (the muscles between the ribs). This is particularly relevant if you have engaged in any physical activity that may have strained these muscles.

4. Gastrointestinal Issues: Conditions affecting the gastrointestinal tract, such as gastritis, peptic ulcers, or even functional gastrointestinal disorders, can cause discomfort in the upper abdomen.
5. Referred Pain: Occasionally, pain or discomfort can be referred from other areas of the body. For instance, issues with the right lung or diaphragm can sometimes present as abdominal discomfort.

Given your symptoms and medical history, it is advisable to take the following steps:
- Consult a Healthcare Provider: Start by visiting a primary care physician or a gastroenterologist. They will be able to assess your symptoms in the context of your medical history and perform a physical examination.

- Diagnostic Tests: Your doctor may recommend several tests to get a clearer picture of what might be causing your discomfort. These could include:
- Ultrasound of the abdomen: This is a non-invasive test that can help visualize the liver, gallbladder, and other abdominal organs.

- Blood tests: Liver function tests can help assess the health of your liver and check for any signs of inflammation or damage.

- CT or MRI scans: If necessary, these imaging studies can provide more detailed information about the abdominal organs.

- Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated factors (like meals or activities). This information can be valuable for your healthcare provider.

In summary, while your discomfort may not seem severe, it is essential to address it, especially given your history of hepatitis B. Early evaluation can help identify any underlying issues and guide appropriate management. Remember, it is always better to err on the side of caution when it comes to health concerns.
