Regarding the management of moles..?
Hello Dr.
Cheng,
I have a larger mole on my body (about the size of a red bean) that is raised and dark in color.
I previously visited the dermatology department at Sin-Lau Hospital in Tainan, but the doctor there advised that this type of mole should be excised by a plastic surgeon.
Due to scheduling conflicts, I would like to return to Taipei for treatment, but I am unsure whether I should see a dermatologist or a plastic surgeon at your hospital for this mole.
Additionally, if it is determined that surgical excision is necessary, will I need to schedule a separate appointment after the initial consultation? Is it possible to have the procedure done on the same day?

Hello: Both dermatology and plastic surgery are options.
Most dermatological procedures can be performed on the same day after an outpatient consultation, while plastic surgery may require a separate appointment.
Sincerely, Dr.

More Info
When it comes to handling large moles, particularly those that are raised and pigmented, there are generally two medical specialties involved: dermatology and plastic surgery. The choice between these specialties often depends on the characteristics of the mole, the patient's medical history, and the desired outcome.
Firstly, it’s important to understand what a mole is. Moles, or nevi, are common skin growths that can vary in size, color, and shape. While most moles are benign, any changes in size, shape, or color, or any new moles that appear after the age of 30 should be evaluated by a healthcare professional. In your case, you mentioned that the mole is about the size of a red bean and raised, which warrants further evaluation.
In dermatology, the primary focus is on diagnosing and treating skin conditions, including moles. Dermatologists can perform excisional biopsies, where the mole is removed entirely along with a margin of surrounding skin. This is often done for diagnostic purposes, especially if there is a concern about skin cancer. If the mole is suspected to be malignant or atypical, a dermatologist is well-equipped to handle the initial assessment and removal.
On the other hand, plastic surgeons specialize in reconstructive and cosmetic procedures. If the mole is large and its removal may result in significant scarring or if it is located in a cosmetically sensitive area (like the face), a plastic surgeon may be preferred. They can provide options for minimizing scarring and may use techniques such as flap reconstruction or grafting if necessary.
Regarding your question about whether to see a dermatologist or a plastic surgeon, it often depends on the specific characteristics of the mole and your personal preferences. If the mole is concerning and you want a thorough evaluation, starting with a dermatologist is a good approach. They can assess the mole and determine if it needs to be removed and whether it should be done by them or referred to a plastic surgeon.
As for scheduling, typically, if a mole requires surgical removal, the initial consultation will involve an assessment and discussion of the procedure. If the decision is made to proceed with the excision, it may not be performed on the same day. This is because the healthcare provider may need to prepare for the procedure, including obtaining consent, preparing the surgical area, and ensuring that all necessary equipment is available. Therefore, it is common to schedule a follow-up appointment for the actual removal.
In summary, handling large moles involves careful consideration of the mole's characteristics and the potential need for surgical intervention. Starting with a dermatologist is advisable for an initial evaluation, and they can guide you on whether a referral to a plastic surgeon is necessary. If surgical removal is indicated, expect to schedule a separate appointment for the procedure itself. Always ensure that you discuss any concerns you have with your healthcare provider to make informed decisions about your skin health.
