Anal Itching: Could It Be Pinworms in Adults? - Surgery

In the early morning, there is often itching around the anal area, as if something is crawling?

Recently, I've been experiencing itching around the anus at night or in the early morning, as if something is crawling.
Even scratching does not alleviate the symptoms.
Could this be pinworms? I tested positive for pinworms in elementary school, but I haven't been tested since.
Aren't pinworms something that only children get? However, I'm already in my second year of college.
This happens every few months and affects my sleep.
Why is this happening? Do pinworms only come to the anus when a person is still? If I stay active and don't sleep at night, will pinworms still crawl to the anus?

Hello: There are many causes of anal pruritus, and parasitic infections are just one of them.
Simple tests, such as stool examination or anal swab microscopy, should be able to determine the presence of a parasitic infection.
It is recommended that you consult a colorectal surgeon for an accurate diagnosis.
In daily life, it is advisable to avoid scratching the affected area, keep the area dry and well-ventilated, and rinse the area with plain water (avoid using soap or body wash).
Additionally, it is best to avoid foods containing caffeine, citrus fruits, tomatoes, and beer, as they can help reduce anal itching.
Wishing you good health.
Sincerely, Dr.

More Info

Anal itching can be a distressing symptom, and your concerns about pinworms (Enterobius vermicularis) are valid, especially given your history of testing positive for them in childhood. While pinworms are most commonly associated with children, adults can also be affected, particularly if they have been in close contact with infected children or contaminated environments.

Understanding Pinworms and Symptoms
Pinworms are small, white, thread-like worms that primarily inhabit the intestines. The female pinworm lays eggs around the anal area, typically at night, which can cause intense itching. This itching is often worse at night, leading to disturbed sleep, as you have experienced. The cycle of pinworm infection can perpetuate itself if the eggs are transferred to the mouth through scratching and then ingested, leading to reinfection.

Can Adults Get Pinworms?
Yes, adults can get pinworms. While it is more common in children due to their behaviors (like playing in dirt and not washing hands thoroughly), adults can contract pinworms through various means, such as:
1. Direct Contact: If you come into contact with someone who is infected, the eggs can be transferred to your hands and then to your mouth.

2. Contaminated Surfaces: Pinworm eggs can survive on surfaces for several weeks. Touching contaminated surfaces and then touching your mouth can lead to infection.

3. Shared Bedding: If you share a bed with someone who is infected, the eggs can be transferred through bedding.

Symptoms of Pinworm Infection
The primary symptom of a pinworm infection is anal itching, particularly at night. Other symptoms may include:
- Irritability or difficulty sleeping due to itching
- Visible worms in the anal area or in stool
- Secondary bacterial infections from scratching

Diagnosis and Treatment
If you suspect you have a pinworm infection, it is essential to consult a healthcare provider. They can confirm the diagnosis through a simple test, often involving the "tape test," where a piece of clear tape is applied to the anal area to collect eggs for examination.

Treatment typically involves over-the-counter or prescription medications such as mebendazole or pyrantel pamoate, which are effective in eliminating the worms. It is crucial to treat all household members simultaneously to prevent reinfection.

Prevention
To prevent future infections, consider the following:
- Good Hygiene: Wash your hands thoroughly with soap and water, especially after using the bathroom and before eating.

- Keep Fingernails Short: This reduces the chance of eggs being trapped under the nails and subsequently ingested.

- Wash Bedding and Clothing: Regularly wash bed linens, pajamas, and towels in hot water to kill any eggs.

- Avoid Scratching: Try to avoid scratching the anal area to prevent the spread of eggs.

Conclusion
While pinworms are often associated with children, adults can certainly be affected. The symptoms you describe, particularly the nocturnal itching, align with a pinworm infection. It is advisable to seek medical attention for proper diagnosis and treatment. Additionally, practicing good hygiene and preventive measures can help reduce the risk of reinfection. If you have ongoing concerns or if symptoms persist, a healthcare professional can provide further guidance and support.
