Presence of sperm in urine?
Hello Doctor, I have a question.
Why do I notice sperm coming out at the end of urination, and the amount is significant, even though I only urinate once a day? I work the night shift, and when I wake up at home, I don't have this issue.
However, when I get to work, the first time I urinate, it happens.
Sometimes, even after I finish urinating, I don't notice it, but when I put on my pants and move, it leaks onto my underwear.
This is very distressing.
Is it due to too much stress, or could it be an infection? After having intercourse with my girlfriend, this situation doesn't occur the next day, but it comes back after a few days.
Since I only urinate once a day, I'm unsure how to approach a doctor about this.
Please help me with your advice.
Thank you! T︿T

Experiencing sperm in urine can be a perplexing and concerning issue, and it’s important to understand the potential causes and implications of this symptom. Based on the information provided, there are several factors to consider that may contribute to this phenomenon.
Firstly, the presence of sperm in urine, known as spermaturia, can occur for various reasons. One common cause is retrograde ejaculation, where semen enters the bladder instead of exiting through the urethra during ejaculation. This can happen due to certain medical conditions, surgeries (such as prostate surgery), or even as a side effect of certain medications. If you have undergone any medical procedures or are taking medications, it would be prudent to discuss these with your healthcare provider.
Another possibility is that the sperm you are noticing in your urine could be residual from a recent ejaculation. If you have had sexual activity or masturbation, it is possible that some sperm may remain in the urethra and be expelled during urination. This can be more noticeable if you are experiencing any form of urethral irritation or inflammation, which could be caused by infections or other urological issues.
In your case, you mentioned that this issue tends to occur more frequently when you are at work, which could suggest that stress or anxiety might be playing a role. Stress can affect various bodily functions, including those related to the urinary and reproductive systems. It can lead to increased muscle tension, which might cause sensations of urgency or discomfort during urination. Additionally, if you are holding your urine for extended periods while at work, this could also contribute to the sensation of sperm being expelled.
You also noted that this issue seems to resolve after sexual activity with your girlfriend, which could indicate that sexual arousal and ejaculation help to clear the urethra of any residual sperm. However, the recurrence of this issue after a few days suggests that there may be an underlying condition that needs to be addressed.
Given the complexity of your symptoms, it is essential to consult with a healthcare professional, preferably a urologist, who can conduct a thorough evaluation. This may include a physical examination, urine tests, and possibly imaging studies to rule out any infections, blockages, or other urological conditions. It is important to communicate all your symptoms, including any pain, discomfort, or changes in urinary habits, as these details can help in diagnosing the underlying cause.
In the meantime, consider implementing some lifestyle changes that may help alleviate your symptoms. Staying well-hydrated, practicing stress management techniques, and avoiding irritants such as caffeine and alcohol may be beneficial. Additionally, if you find that certain activities or positions exacerbate your symptoms, it may be helpful to modify those behaviors until you can see a doctor.
In conclusion, while experiencing sperm in your urine can be concerning, it is often manageable with the right medical guidance. Do not hesitate to seek professional help to ensure that any potential underlying issues are addressed appropriately. Your health and well-being are paramount, and a urologist will be best equipped to provide you with the answers and treatment you need.
