Chest tightness and diarrhea?
Hello Doctor: I would like to inquire about a news report I came across stating that "chest tightness and diarrhea may indicate a myocardial infarction." I have noticed that I experience similar symptoms.
Occasionally, I feel a tightness in my chest, and sometimes I have to take a deep breath, which mostly occurs in cold weather.
As for diarrhea, that happens more frequently.
Could you please explain the possible reasons for these symptoms? Thank you~~

Dr. Lin Liangyu reply Cardiology
Hello: Myocardial infarction can present with abdominal symptoms, typically due to excessive stimulation of the parasympathetic nervous system caused by inferior wall infarction.
The primary symptoms are usually nausea and vomiting, while diarrhea is less common and more frequently observed in the elderly.
In younger individuals, myocardial infarction primarily presents with chest pain and chest discomfort.
Your condition is not indicative of a myocardial infarction; it is more likely a functional gastrointestinal disorder.
Wishing you a speedy recovery.

Chest tightness and diarrhea can indeed be concerning symptoms, especially when they occur together. While it is essential to consider various potential causes, including gastrointestinal issues, anxiety, or even respiratory problems, the possibility of a heart attack (myocardial infarction) should not be overlooked, particularly if there are risk factors present.
Chest tightness can be a classic symptom of a heart attack, often described as a feeling of pressure, squeezing, or fullness in the chest. It may also radiate to the arms, back, neck, jaw, or stomach. In some cases, individuals may experience atypical symptoms, especially women, who may present with less common signs such as shortness of breath, nausea, or even gastrointestinal discomfort. The presence of diarrhea, while not a typical symptom of a heart attack, can occur in some individuals due to the body's stress response or as a result of vagus nerve stimulation, which can affect both the heart and the gastrointestinal tract.
In your case, the combination of chest tightness and diarrhea could be indicative of several underlying issues. It is crucial to consider your overall health, including any risk factors for coronary artery disease (CAD) such as age, family history, smoking, hypertension, diabetes, and hyperlipidemia. If you have any of these risk factors, it increases the importance of evaluating your symptoms further.
Additionally, anxiety and stress can manifest physically, leading to symptoms such as chest tightness and gastrointestinal disturbances, including diarrhea. This is often referred to as psychosomatic symptoms, where emotional distress translates into physical symptoms. Cold weather can also exacerbate anxiety or respiratory issues, leading to a feeling of tightness in the chest.
Given your symptoms, it is advisable to consult with a healthcare professional who can perform a thorough evaluation. This may include a physical examination, an electrocardiogram (ECG), blood tests to check for cardiac enzymes, and possibly imaging studies such as a chest X-ray or echocardiogram to assess heart function. If there is any suspicion of a heart attack or significant cardiac issues, further testing such as a stress test or coronary angiography may be warranted.
In summary, while chest tightness and diarrhea can arise from various causes, including anxiety or gastrointestinal issues, they can also signal a potential cardiac event, especially if accompanied by other risk factors. It is essential to seek medical attention promptly to rule out any serious conditions and to receive appropriate care based on a comprehensive assessment of your symptoms and health history. Remember, it is always better to err on the side of caution when it comes to heart health.
