Concerns After Circumcision: Is My Surgery Successful? - Urology

Just had a circumcision?

Hello Doctor, I underwent a circumcision yesterday due to a narrow foreskin opening.
This morning, I woke up to find that the glans, which was previously exposed, has retracted back under the foreskin, with only a small portion visible.
I can push the foreskin back to the state it was immediately after the surgery, but whenever I bend over or squat, it retracts again.
Is this a sign of surgical failure? Additionally, I weigh nearly 100 kilograms, which may contribute to a shorter penis.
Will I need another surgery? Can the wound come into contact with water? Thank you!

Hello: That is because your fat layer is too thick.
It is best to discuss this in detail with your surgical physician.
Sincerely, Dr.

After undergoing a circumcision procedure, it is understandable to have concerns about the success of the surgery, especially if you are experiencing unexpected changes in the position of the glans (the head of the penis). Based on your description, it seems that the glans is retracting back under the remaining foreskin when you bend or squat, which can be concerning. However, let's break down your concerns and provide some clarity.

1. Understanding the Surgery: Circumcision is performed to remove the foreskin, which can sometimes be too tight (phimosis) or for other medical reasons. The goal is to expose the glans fully and allow for better hygiene and comfort. After surgery, it is normal for some swelling and healing to occur, which can affect how the glans appears and feels.

2. Glans Positioning: If the glans is retracting back under the foreskin, it could be due to swelling or the natural healing process. In some cases, the skin may not have fully settled into its new position immediately after surgery. However, if the glans is consistently retracting and you are unable to keep it exposed, it may indicate that the circumcision did not remove enough foreskin or that there is some other complication.

3. Weight Considerations: You mentioned that you weigh close to 100 kg, which could potentially affect the anatomy and positioning of the penis. In some cases, excess weight can lead to changes in how the skin and tissues behave, but this is not typically a direct cause of surgical failure. It is important to maintain a healthy weight for overall health, which can also impact recovery.

4. Water Exposure: After circumcision, it is generally advised to keep the area dry and clean for a few days. You should avoid soaking in water (like baths or swimming) until the wound has healed sufficiently, which usually takes about a week. Showering is typically acceptable, but be gentle and avoid direct water pressure on the surgical site.

5. When to Seek Help: If you continue to experience issues with the glans retracting, or if you notice signs of infection (such as increased redness, swelling, or discharge), it is crucial to follow up with your surgeon or a urologist. They can assess whether the surgery was successful and if any further intervention is necessary.

6. Potential for Revision Surgery: If it is determined that the circumcision did not achieve the desired outcome, a revision surgery may be considered. This is typically a straightforward procedure, but it is essential to discuss the risks and benefits with your healthcare provider.

In conclusion, while it is normal to have concerns after surgery, especially regarding the healing process, it is essential to monitor your symptoms and communicate with your healthcare provider. They can provide the best guidance based on your specific situation and ensure that you receive the appropriate care. Remember, healing takes time, and it is crucial to give your body the opportunity to recover fully before making any decisions about further procedures.

If there is no impact on erection due to a tight foreskin, it should not be a concern. This aspect should be communicated clearly with the physician before the procedure, as discussing it afterward would be of no use.
