Improving scars
Dear Dr.
Lu,
When I was a child, out of curiosity, I often bit the skin on the back of my left hand, specifically around the middle finger area.
A portion of the tissue has necrosed and appears a completely different color from the surrounding skin, seemingly due to blood vessels, which gives it a reddish hue.
The necrotic area is located at the center of the back of my left hand, at the middle finger, and is smaller than a one-dollar coin, but it is very noticeable, which makes me feel quite sad.
Is there a surgical option to improve this condition? What would the approximate cost be? Thank you for taking the time to answer my question despite your busy schedule.
I appreciate it!
Best regards.

Scars smaller than one centimeter can theoretically be excised, but it is necessary to first assess the original condition of the scar to determine which treatment method is more appropriate.

More Info
When it comes to improving the appearance of scars, especially those resulting from childhood injuries or habits, there are several effective surgical and non-surgical options available. Given your description of the scar on your left hand, it seems that the area has undergone significant changes, including color differences and possibly some tissue loss. Here’s a comprehensive overview of the options you might consider, along with their potential benefits and risks.
Surgical Options
1. Scar Revision Surgery: This is a common procedure where a surgeon removes the scar tissue and repositions the skin to create a more aesthetically pleasing appearance. The goal is to make the scar less noticeable. The effectiveness of this procedure largely depends on the size, location, and type of scar. Since your scar is relatively small, this could be a viable option.
2. Skin Grafting: If the scar involves significant tissue loss, a skin graft might be necessary. This involves taking a piece of skin from another part of your body and placing it over the scarred area. This can help restore a more uniform skin tone and texture.
3. Laser Therapy: While not a surgical option in the traditional sense, laser treatments can significantly improve the appearance of scars. Laser therapy works by removing the outer layer of skin or stimulating collagen production in the deeper layers. This can help reduce redness and improve texture.
4. Microneedling: This minimally invasive procedure involves using fine needles to create micro-injuries in the skin, promoting collagen production and improving the appearance of scars over time.
Non-Surgical Options
1. Topical Treatments: There are various creams and ointments available that can help improve the appearance of scars. These often contain ingredients like silicone, which can help flatten and soften scars over time.
2. Chemical Peels: This treatment involves applying a chemical solution to the skin, which causes the top layer to peel off, revealing newer, less damaged skin underneath. This can help improve the color and texture of the scar.
3. Dermal Fillers: For scars that are indented or have lost volume, dermal fillers can be injected to raise the scar to the level of the surrounding skin.
Considerations and Costs
- Consultation: It’s essential to consult with a board-certified dermatologist or plastic surgeon who specializes in scar treatment. They can assess your specific situation and recommend the best course of action tailored to your needs.
- Costs: The cost of scar treatment can vary widely based on the type of procedure, the surgeon’s experience, and your geographical location. For example, scar revision surgery can range from $500 to $3,000, while laser treatments might cost between $200 to $1,500 per session. Insurance may cover some procedures if deemed medically necessary, so it’s worth checking with your provider.
- Recovery and Risks: Each procedure comes with its own set of risks and recovery times. Surgical options may require downtime and can lead to complications such as infection or additional scarring. Non-surgical options typically have shorter recovery times but may require multiple sessions for optimal results.
Conclusion
Improving the appearance of your scar is certainly possible through various surgical and non-surgical methods. The best approach will depend on the specific characteristics of your scar and your personal preferences. A thorough consultation with a qualified professional will help you understand the most effective options available to you, along with their associated costs and recovery times. Remember, it’s important to have realistic expectations and to discuss any concerns you may have with your healthcare provider.
