After healing external hemorrhoids, anal itching may occur?
Hello Dr.
Hou,
Two weeks ago, I developed an external hemorrhoid at the anal area.
A surgeon prescribed me a topical medication called Proctosedyl, and after about seven to eight days, the external hemorrhoid has disappeared and looks normal.
However, I still have a persistent itching sensation around the anus.
How can I resolve this? Is it a side effect of the treatment? Thanks!

1.
Anal itching is a separate condition with many potential causes.
It is generally not a side effect of treatment, so please consult a colorectal specialist for diagnosis and treatment.

Managing post-hemorrhoid surgery itching can be a common concern for patients, and understanding the underlying causes and potential solutions is essential for effective management. Itching after hemorrhoid surgery, or even after conservative treatment for hemorrhoids, can arise from several factors.
Causes of Itching After Hemorrhoid Surgery
1. Healing Process: After surgery, the body goes through a natural healing process. As the surgical site heals, it may become irritated, leading to itching. This is often a normal part of recovery.
2. Skin Irritation: The area around the anus is sensitive, and any surgical intervention can lead to irritation. Factors such as moisture, friction from clothing, or even the use of topical medications can contribute to skin irritation and itching.
3. Infection: Although rare, an infection at the surgical site can cause itching, redness, swelling, and discharge. If you notice any signs of infection, it is crucial to contact your healthcare provider.
4. Allergic Reaction: Some patients may develop an allergic reaction to topical medications, such as the ointment prescribed (保痔寧). This can lead to localized itching and discomfort.
5. Hemorrhoid Recurrence: In some cases, if the underlying issues that caused hemorrhoids initially are not addressed, patients may experience a recurrence of hemorrhoids, which can also lead to itching.
Solutions for Managing Itching
1. Keep the Area Clean and Dry: Maintaining good hygiene is crucial. Gently clean the area with mild soap and water, and ensure it is thoroughly dried afterward. Avoid using harsh soaps or scrubbing the area, as this can exacerbate irritation.
2. Use Moisture-Wicking Fabrics: Wearing breathable, moisture-wicking underwear can help keep the area dry and reduce irritation. Cotton is often recommended for its breathability.
3. Topical Treatments: If the itching persists, over-the-counter hydrocortisone cream may provide relief. However, it is essential to consult with your healthcare provider before using any new topical treatments to ensure they are appropriate for your situation.
4. Avoid Scratching: While it may be tempting to scratch the itchy area, doing so can lead to further irritation or even infection. Instead, try to distract yourself or use cold compresses to alleviate the itching sensation.
5. Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation, which can strain the anal area and exacerbate itching. Staying hydrated is also essential for maintaining bowel regularity.
6. Follow-Up with Your Doctor: If the itching does not improve or worsens, it is crucial to follow up with your healthcare provider. They can assess the situation, rule out any complications, and provide additional treatment options if necessary.
Conclusion
Itching after hemorrhoid surgery can be bothersome but is often manageable with proper care and attention. Understanding the potential causes and implementing effective solutions can significantly improve comfort during the recovery process. If symptoms persist or worsen, seeking medical advice is essential to ensure there are no underlying complications. Always prioritize communication with your healthcare provider for personalized care and recommendations tailored to your specific situation.
