Varicocele
Hello doctor, it's been a month since my varicocele surgery, but sometimes after standing for 2 to 3 hours, I still experience some pain.
Why is this happening? Is it a recurrence?

Hello, I think you are too anxious.
It has only been one month since the surgery, and the wound is just beginning to heal.
For varicose vein surgery, most commonly a high ligation technique is used, which means the incision is not located on the scrotum.
This type of incision tends to be less painful and the surgery duration is shorter.
However, the downside is that it takes about six months for the varicose veins to return to normal, meaning that it will take approximately five months for the veins to return to their normal size.
Of course, the level of pain will not disappear immediately; it will gradually improve over about 2-3 months.
Note: Semen analysis will also gradually return to normal around two and a half months post-surgery.
Urology Department Chief, Hsu Wei-Kai, responding on behalf of the Taitung Hospital, Department of Health, Executive Yuan, is concerned about you.

After undergoing varicocele repair surgery, it is not uncommon to experience some degree of discomfort or pain during the recovery period. The surgery typically involves ligating the affected veins to redirect blood flow and alleviate the symptoms associated with varicocele, such as pain or swelling in the scrotum. While many patients report significant improvement in their symptoms post-surgery, some may still experience lingering discomfort, especially when standing for extended periods.
The pain you are experiencing after standing for two to three hours could be attributed to several factors. First, it is important to recognize that the surgical site is still healing, and the tissues may be sensitive or inflamed. The healing process can take several weeks to months, and during this time, it is normal to feel some discomfort, particularly with activities that put strain on the area, such as prolonged standing.
Additionally, the pain may not necessarily indicate a recurrence of the varicocele. Post-operative pain can stem from nerve irritation, muscle strain, or even the body adjusting to the changes made during surgery. It is also possible that the pain is related to the formation of scar tissue, which can occur as the body heals. Scar tissue can sometimes lead to sensations of tightness or discomfort in the area.
If the pain is mild and manageable, it may simply be a part of the normal recovery process. However, if you notice an increase in pain intensity, swelling, or any other concerning symptoms such as fever or changes in urination, it is crucial to consult your surgeon or healthcare provider. They may recommend imaging studies, such as an ultrasound, to rule out any complications, including the possibility of a recurrent varicocele or other issues.
In the meantime, here are some general recommendations for managing post-surgical discomfort:
1. Rest and Activity Modification: Allow your body time to heal by balancing rest with gentle activity. Avoid standing for long periods and take breaks to sit or lie down when needed.
2. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help alleviate discomfort. Always follow your doctor's recommendations regarding medication.
3. Supportive Garments: Wearing supportive underwear may provide comfort and help reduce strain on the surgical site.
4. Follow-Up Appointments: Keep all scheduled follow-up appointments with your surgeon. These visits are essential for monitoring your recovery and addressing any concerns you may have.
5. Listen to Your Body: Pay attention to your body’s signals. If certain activities exacerbate your pain, it may be wise to modify those activities until you feel more comfortable.
In conclusion, while some post-operative pain after varicocele repair is normal, it is essential to monitor your symptoms and communicate with your healthcare provider if you have concerns. They can provide personalized advice and reassurance based on your specific situation, ensuring a smooth recovery process.
