Itching in the genital area?

Hello, doctor.
I have been experiencing itching in my genital area for a long time.
The first time was in my sophomore year when I visited a gynecologist, took medication, and applied ointment, which resolved the issue without recurrence.
However, in my senior year, it started to recur frequently.
Previously, it would happen every 2 to 3 months, but now it occurs almost once a month.
I have been seeing a gynecologist regularly, and this time I felt discomfort again.
The doctor mentioned that the recurrence is too frequent and found some pustules on my cervix.
They advised me to go to a larger hospital for further examination, which made me very anxious.

Could you please explain why a small gynecology clinic cannot perform the necessary examinations and why I need to go to a larger hospital? Am I at risk for cervical cancer? I am 24 years old and have a steady sexual partner.
I really don't understand why I am experiencing this itching so easily.
I live in Taichung.
Could you please advise me on where to go for a more reliable examination? Thank you.

vivian, 20~29 year old female. Ask Date: 2005/07/04

Dr. Li Guanghan reply Obstetrics and Gynecology

Hello: Itching in the genital area may be due to a Candida infection, and cervical mucous cysts could be physiological Nabothian cysts.
To rule out cervical cancer, a pelvic examination and Pap smear are necessary.
Please seek medical attention promptly; you can go to Taichung Veterans General Hospital for evaluation or visit the Obstetrics and Gynecology Department at Hsinchu City Hospital.

Persistent vaginal itching can be a distressing issue, and it’s understandable that you are seeking clarity on the causes and next steps. Based on your description, it seems that you have experienced recurrent episodes of itching and discomfort, which have prompted visits to your gynecologist. Here’s a comprehensive overview of potential causes, diagnostic steps, and treatment options.

Causes of Persistent Vaginal Itching
1. Infections: The most common cause of vaginal itching is infections, which can include:
- Yeast Infections: Caused by an overgrowth of Candida, leading to itching, burning, and discharge.

- Bacterial Vaginosis: An imbalance of normal bacteria in the vagina, often resulting in a fishy odor and irritation.

- Sexually Transmitted Infections (STIs): Conditions like trichomoniasis or herpes can cause itching and discomfort.

2. Skin Conditions: Conditions such as eczema or lichen sclerosus can lead to itching and irritation in the vaginal area.

3. Hormonal Changes: Fluctuations in hormones, particularly during menstruation, pregnancy, or menopause, can lead to dryness and itching.

4. Allergic Reactions: Sensitivities to soaps, detergents, or personal hygiene products can cause irritation.

5. Other Factors: Stress, poor hygiene, and wearing tight or non-breathable clothing can also contribute to vaginal itching.

Diagnostic Steps
Given your symptoms and the recommendation from your doctor to visit a larger hospital for further evaluation, it’s crucial to understand that more comprehensive testing may be necessary. This could include:
- Pelvic Examination: A thorough examination to check for signs of infection or other abnormalities.

- Pap Smear: To screen for cervical cancer and detect any abnormal cells.

- Cultures or Swabs: To identify specific infections, including yeast or bacterial infections.

- Blood Tests: To check for STIs or other underlying conditions.

Concerns About Cervical Cancer
Your concern about cervical cancer is valid, especially since you mentioned the presence of “膿泡” (pustules) on the cervix. While cervical cancer is a possibility, it is essential to remember that many conditions can cause similar symptoms. Regular Pap smears and HPV vaccinations are effective preventive measures against cervical cancer. At 24 years old, it’s important to stay vigilant about your reproductive health, but it’s also crucial not to jump to conclusions without proper testing.

Next Steps
1. Follow-Up with a Specialist: Since your gynecologist has recommended further evaluation at a larger hospital, it’s advisable to follow through with that. Specialists in larger facilities often have access to advanced diagnostic tools and treatments.

2. Maintain Open Communication: Discuss your symptoms in detail with your healthcare provider. Be honest about your sexual history, any changes in your health, and your concerns regarding cervical cancer.

3. Consider Lifestyle Modifications: In the meantime, consider factors that may contribute to your symptoms. This includes wearing breathable cotton underwear, avoiding scented products, and practicing good hygiene.

4. Stay Informed: Educate yourself about potential conditions that could be causing your symptoms. Understanding your body can empower you to make informed decisions regarding your health.

Conclusion
Persistent vaginal itching can stem from various causes, and it’s essential to approach this issue with a thorough understanding and appropriate medical guidance. By following your doctor’s recommendations for further evaluation and staying proactive about your health, you can work towards finding relief and ensuring your reproductive health is well-managed. Don’t hesitate to seek support from healthcare professionals who can provide the care you need.
