Please provide your questions regarding hemorrhoids?

Hello, doctor.
I have been suffering from hemorrhoids for many years.
During my military service, the significant change in environment caused me to feel stressed and anxious, leading to a period where I didn't have a bowel movement for almost a week.
After that, I started experiencing difficulty in bowel movements, pain in the anal area, inflammation, and bleeding.
This condition has persisted for 6 to 7 years and has caused me considerable trouble at work.
For 3 to 4 years, I visited other hospitals several times, but the doctors said it was manageable and advised me to rely on medication and dietary control.
However, as the years went by, I only noticed that the condition worsened, and I don't feel any signs of improvement, possibly due to work-related stress.
Currently, I have bowel movements approximately every 3 to 4 days, and after each bowel movement, I experience pain for 1 to 2 days.
Therefore, I suffer from symptoms for more than 2 days each week.
The symptoms resemble both internal and external hemorrhoids, with cracking pain and a feeling of pressure.
Sometimes, inflammation leads to bleeding during bowel movements.
Whenever I experience inflammation for 1 to 2 days, there is always bleeding, which is very distressing.
I am unsure about the severity of my condition, but if I go more than 4 days without a bowel movement, I end up with diarrhea.
The stool is hard at the front and liquid at the back.
I hope you can provide some advice! Thank you!

Hemorrhoids, commonly known as piles, are swollen veins in the lower rectum and anus that can cause discomfort, pain, and bleeding. Understanding the causes, symptoms, and treatment options for hemorrhoids is essential for managing this condition effectively.

Causes of Hemorrhoids
Hemorrhoids can develop due to various factors, including:
1. Straining During Bowel Movements: This is often caused by constipation or diarrhea. When you strain to pass stool, it puts pressure on the veins in the rectal area, leading to swelling.

2. Prolonged Sitting: Sitting for long periods, especially on the toilet, can increase pressure on the veins in the anus.

3. Pregnancy: The weight of the fetus can put pressure on the pelvic veins, leading to hemorrhoids.

4. Obesity: Excess weight can increase pressure on the pelvic veins.

5. Aging: As you age, the tissues supporting the veins in the rectum and anus can weaken, making hemorrhoids more likely.

6. Chronic Coughing or Sneezing: Conditions that cause chronic coughing or sneezing can increase pressure in the abdominal area, contributing to hemorrhoid formation.

Symptoms of Hemorrhoids
The symptoms of hemorrhoids can vary depending on their type (internal or external) and severity. Common symptoms include:
- Pain or Discomfort: This is particularly noticeable during bowel movements or when sitting.

- Bleeding: Bright red blood may appear on toilet paper or in the toilet bowl after a bowel movement.

- Swelling: You may notice swelling around the anus.

- Itching or Irritation: This can occur around the anal area.

- Lumps: You may feel lumps near the anus, which can be sensitive or painful.

Treatment Options
Managing hemorrhoids often involves lifestyle changes and home remedies, but in some cases, medical treatment may be necessary. Here are some options:
1. Dietary Changes: Increasing fiber intake can help soften stools and reduce straining. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying hydrated is also crucial.

2. Over-the-Counter Medications: Topical treatments, such as creams or ointments containing hydrocortisone or witch hazel, can relieve itching and discomfort. Oral pain relievers like acetaminophen or ibuprofen can help manage pain.

3. Warm Baths: Soaking in a warm bath for 10-15 minutes several times a day can help reduce swelling and relieve discomfort.

4. Avoid Straining: Try not to strain during bowel movements. If you're having difficulty, consider using a stool softener or laxative as recommended by a healthcare provider.

5. Surgical Options: If conservative treatments fail, procedures such as rubber band ligation, sclerotherapy, or hemorrhoidectomy may be necessary. These options are typically considered for severe cases or when hemorrhoids are recurrent.

When to Seek Medical Attention
Given your symptoms, it is essential to consult a healthcare provider, especially if you experience:
- Persistent pain or discomfort.

- Significant bleeding.

- Changes in bowel habits.

- Symptoms that do not improve with home treatment.

Conclusion
Hemorrhoids can significantly impact your quality of life, especially when they cause pain and discomfort. It is crucial to address the underlying causes, such as dietary habits and bowel movement regularity, to manage symptoms effectively. If conservative measures do not provide relief, do not hesitate to seek medical advice for further evaluation and treatment options. Remember, early intervention can prevent complications and improve your overall well-being.
