Thinner retina?
I have high myopia of over 1000 degrees, and regular check-ups have shown that my retina is thinner and I experience symptoms of floaters.
Will straining during childbirth have any impact or cause harm to my eyes?
Yijun, 20~29 year old female. Ask Date: 2001/06/21
Dr. Huang Bozhen reply Ophthalmology
According to page 505 of GIIYER's Retina-Vitreous-Macula, a study involving 50 pregnant women with high myopia showed that there were no cases of retinal detachment occurring before or after delivery.
Additionally, 13 pregnant women who had previously undergone retinal detachment surgery prior to pregnancy also did not experience any detachment after natural childbirth.
Conclusion: There is no need to fear that excessive straining during delivery will cause eye damage, and therefore, cesarean delivery is not necessarily required.

When it comes to the impact of straining during labor on eye health, particularly for individuals with high myopia (over 1000 degrees) and a thin retina, there are several important considerations to keep in mind.
Firstly, high myopia is associated with an increased risk of various ocular complications, including retinal detachment, macular degeneration, and other vision-threatening conditions. The thinness of the retina in individuals with high myopia can make it more susceptible to damage, especially during activities that involve significant physical exertion, such as labor.
During labor, especially when pushing, there is a significant increase in intraocular pressure (IOP). This increase can potentially exacerbate existing retinal issues. For someone with a thin retina, the strain could theoretically increase the risk of retinal tears or detachment. While the body is designed to handle normal levels of exertion, the added pressure during labor can pose a risk for those with pre-existing eye conditions.
It is crucial to discuss your specific situation with your healthcare provider, particularly an ophthalmologist who understands your eye health history. They can provide personalized advice based on the condition of your retina and overall eye health. In many cases, doctors may recommend avoiding excessive straining during labor, and they might suggest alternative delivery methods if they believe that pushing could pose a risk to your vision.
Additionally, if you experience any sudden changes in vision, such as flashes of light, an increase in floaters, or a shadow in your peripheral vision during or after labor, it is essential to seek immediate medical attention. These symptoms could indicate a retinal tear or detachment, which requires prompt treatment to prevent permanent vision loss.
In terms of preventive measures, it may be beneficial to engage in prenatal care that includes discussions about your eye health. Your obstetrician can work closely with your ophthalmologist to create a delivery plan that minimizes risks to your eyes. This may include strategies for managing labor that reduce the need for excessive straining.
Moreover, maintaining regular check-ups with your eye doctor is essential, especially as you approach labor. They can monitor any changes in your retina and provide guidance on how to protect your vision during this critical time.
In summary, while straining during labor can potentially impact eye health, particularly for those with high myopia and a thin retina, the degree of risk varies from person to person. Open communication with your healthcare providers and careful monitoring of your eye health can help mitigate potential risks and ensure a safer delivery experience. Always prioritize your vision health alongside your overall well-being during pregnancy and labor.
