Please help me, doctor..?
Hello, Doctor! I would like to ask a question on behalf of my family.
My father is currently fifty years old and generally healthy, but he has recently developed a symptom that even doctors at large hospitals cannot diagnose (blood tests have all come back normal).
His symptoms include occasional swelling of the palms and backs of his hands (the entire palm), peeling skin, redness with slight burning sensation, and itching.
Sometimes it improves, but then it recurs.
I am quite anxious and have encouraged him to seek medical attention, but both large and small hospitals, as well as traditional Chinese medicine practitioners, have not yielded any results.
Therefore, I would like to ask you what kind of symptoms these might be? Also, are there any reputable large hospitals in the northern region? Which one would you recommend (or which doctor)? Thank you very much!

Based on your description, your father's condition may be a form of angioedema, which is a deeper form of urticaria.
There are many potential triggers, similar to urticaria, including food, medications, emotional stress, and temperature changes.
It is important to identify the triggers from daily life and manage the condition with medication for a period of time.
Additionally, reducing the intake of foods containing colorings, preservatives, and other additives may help improve the urticaria symptoms.
There are many large hospitals in the northern region, and he can seek medical attention as needed.

More Info
It sounds like your father is experiencing a complex dermatological issue that has not been easily diagnosed despite consultations with various healthcare providers. The symptoms you describe—swelling of the palms, peeling skin, redness, a burning sensation, and itching—can be indicative of several skin conditions, including eczema, contact dermatitis, or even a form of chronic urticaria (hives).
Eczema, or atopic dermatitis, is a common condition that can cause dry, itchy, and inflamed skin. It often flares up in response to environmental factors such as allergens, irritants, or changes in temperature and humidity. Contact dermatitis occurs when the skin reacts to a substance it has come into contact with, leading to similar symptoms. Chronic urticaria can also present with swelling and redness, often triggered by stress, certain foods, or environmental factors.
Given that your father's symptoms are intermittent and can resolve on their own, it might suggest a condition that is influenced by external factors. Stress, changes in weather, or exposure to certain materials (like soaps, detergents, or even fabrics) could be contributing to his symptoms.
In terms of treatment, it is essential to identify and avoid any potential triggers. Keeping the skin moisturized is crucial, as dry skin can exacerbate these conditions. Over-the-counter topical corticosteroids can help reduce inflammation and itching, but they should be used under the guidance of a healthcare professional, especially for prolonged periods.
Regarding your question about seeking further medical help, if your father has not found relief or a diagnosis from local hospitals, it may be beneficial to consult a dermatologist who specializes in complex skin conditions. In the northern part of Taiwan, hospitals like National Taiwan University Hospital or Taipei Veterans General Hospital have reputable dermatology departments. It may also be worthwhile to seek a second opinion from a different dermatologist who may have a fresh perspective on his symptoms.
In addition to dermatological evaluation, it might be helpful to consider a comprehensive approach that includes allergy testing, as some skin conditions can be exacerbated by allergic reactions. A dermatologist may recommend patch testing to identify any allergens that could be causing contact dermatitis.
Lastly, keeping a symptom diary can be beneficial. Documenting when the symptoms occur, any potential triggers, and the effectiveness of treatments can provide valuable information to healthcare providers and help in diagnosing the condition.
In summary, while your father's symptoms are concerning and warrant further investigation, there are several potential explanations. A thorough evaluation by a dermatologist, possibly at a specialized hospital, along with a focus on identifying triggers and maintaining skin hydration, will be key steps in managing his condition.
