How to clean the foreskin?

Dear Doctor: I wash my foreskin every day, but I still feel like there is a lot of smegma.
I use my hands to scrub until it's clean, and then I wash with body wash.
Is this the correct way to clean? However, recently I've noticed that my glans and foreskin feel very dry, and the area around the coronal sulcus and foreskin seems to have become thinner.
After having intercourse, I also notice some cracks in the coronal sulcus and foreskin area.
What could be the reason for this? Doctor, am I cleaning correctly? If not, how should I properly clean and care for this area? Thank you, Doctor.

This is called an action injury! Do you wash your face like this too? Be gentle while showering, and for the laceration, please consult a urologist.

When it comes to proper techniques for cleaning the foreskin, it's essential to understand both the anatomy involved and the best practices for hygiene. The foreskin, or prepuce, is a fold of skin that covers the glans (tip) of the penis. It can accumulate smegma, a combination of dead skin cells, oils, and other bodily secretions, which can lead to hygiene issues if not cleaned properly.

First and foremost, it's important to approach cleaning the foreskin gently. Using your hands to rub vigorously can irritate the sensitive skin of the glans and the inner foreskin, leading to dryness and potential micro-tears. Instead of scrubbing hard, you should aim to gently retract the foreskin and rinse the area with warm water. If you feel the need to use soap, opt for a mild, unscented soap to avoid irritation. Harsh soaps or body washes can strip the natural oils from the skin, leading to dryness and discomfort.

Here are some steps to follow for proper foreskin hygiene:
1. Gentle Retraction: When cleaning, gently pull back the foreskin to expose the glans. This should be done carefully to avoid any discomfort or pain.

2. Rinse with Water: Use warm water to rinse the area. This helps to remove smegma without the need for aggressive scrubbing.

3. Mild Soap (if necessary): If you feel that water alone is not sufficient, you can use a mild, unscented soap. Apply a small amount to your fingertips and gently clean the area. Rinse thoroughly to ensure no soap residue remains.

4. Drying: After cleaning, gently pat the area dry with a clean, soft towel. Avoid using rough materials that could irritate the skin. If you are concerned about lint from towels, consider using a dedicated soft cloth or a cotton pad.

5. Moisturizing: If you experience dryness, consider applying a small amount of a fragrance-free moisturizer or a specialized product designed for sensitive skin. This can help maintain moisture and prevent irritation.

6. Avoiding Irritants: Be cautious with products that may irritate the area, such as heavily scented lotions, powders, or lubricants. If you notice any allergic reactions or irritation, discontinue use immediately.

Regarding the concerns about the thinning of the foreskin and the presence of cracks after sexual activity, these symptoms could indicate irritation or an underlying condition such as balanitis (inflammation of the glans) or a skin condition. It’s crucial to consult a healthcare professional if you experience persistent dryness, pain, or any unusual symptoms. They can provide a proper diagnosis and recommend appropriate treatments, which may include topical medications or lifestyle adjustments.

In summary, proper foreskin hygiene involves gentle cleaning with warm water, the occasional use of mild soap, and careful drying. Avoid aggressive scrubbing and be mindful of any products that may cause irritation. If you have ongoing issues with dryness or discomfort, seeking medical advice is essential to ensure your health and well-being.
