Red spots on the glans?
I have many small red bumps on my glans that are slightly raised.
They are not painful or itchy, but they have been increasing in number recently.
I visited a hospital in Tainan last Saturday, but the doctor said they didn't know the cause and advised me to maintain good hygiene.
Could these symptoms be a sign of syphilis? I have had sexual intercourse, but I always use condoms and have a steady partner.
Thank you, doctor.

Recently, I came across a case where the patient reported having no sexual experience, making it difficult to assess the true cause.
However, since you have a habit of using condoms, the red spots on the glans are unlikely to be indicative of syphilis.
They could potentially be caused by a drug allergy or an allergy to additives in the condom.
You might consider stopping the use of condoms for a while or trying a different brand to see if there is any improvement.

Red bumps on the glans can be concerning, especially when they appear suddenly or increase in number. While I understand that you have already consulted a physician who advised you to maintain good hygiene, it’s important to explore the potential causes and implications of these symptoms further.
First, let's consider the characteristics of the bumps. Since you mentioned that they are small, red, and not painful or itchy, this could suggest a few possibilities. Common benign conditions that could cause such symptoms include:
1. Pearly Penile Papules: These are small, harmless bumps that often appear around the corona of the glans. They are a normal anatomical variant and are not associated with any disease.
2. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands that are visible on the surface of the skin. They are completely normal and do not require treatment.
3. Folliculitis: This is an inflammation of the hair follicles that can occur due to irritation, infection, or blockage. It may present as small red bumps and is usually self-limiting.
4. Allergic Reactions: Sometimes, contact dermatitis from soaps, lotions, or even latex from condoms can cause localized bumps or irritation.
5. Sexually Transmitted Infections (STIs): While you mentioned that you have been using condoms and have a fixed partner, it’s still important to consider STIs as a potential cause. Conditions such as genital warts (caused by HPV) or molluscum contagiosum can present as bumps on the genital area. Syphilis can also cause lesions, but these typically have other accompanying symptoms.
Regarding your concern about syphilis, it is crucial to note that the primary stage of syphilis often presents with a painless sore (chancre) rather than small red bumps. However, if you have had unprotected sexual contact or if your partner has had any unexplained symptoms, it would be prudent to get tested for STIs, including syphilis, regardless of the use of condoms.
Given that your symptoms have persisted and increased in number, it would be advisable to seek a second opinion from a dermatologist or a urologist who specializes in genital conditions. They may perform a physical examination and possibly recommend a biopsy or other tests to rule out any underlying conditions.
In the meantime, continue to maintain good hygiene, avoid any irritants, and refrain from sexual activity until you have a clearer understanding of the cause of your symptoms. If you notice any changes such as pain, discharge, or the appearance of sores, seek medical attention promptly.
In summary, while many causes of red bumps on the glans are benign, it is essential to monitor your symptoms closely and consult a specialist if they persist or worsen. Regular STI screenings are also a good practice, especially if there are any changes in your sexual health or relationship dynamics. Your health and peace of mind are paramount, so don’t hesitate to advocate for yourself in seeking further evaluation.
