It's been over three months since my varicocele surgery, but sometimes after an erection that lasts too long, I still experience pain afterward.
What could be the reason for this? Is it a recurrence?

Hello, sometimes prolonged erections can cause muscle soreness on their own and are not necessarily related to varicocele.
You can check for recurrence by looking for small vascular networks above the testicles.
