Varicocele
Hello Doctor, I would like to ask about my condition.
I underwent high ligation of the spermatic vein for varicocele at the end of June.
I felt a significant improvement right after the surgery, but over the past two to three days, I've started to experience a dull ache in my left testicle again.
This discomfort does not improve when lying down, but when I palpate the spermatic vein, it does not feel as enlarged or prominent as before.
Could this indicate a possible recurrence, or could there be another underlying cause? Before the surgery, I also did not experience relief when lying down, which makes me wonder if it has recurred.
Thank you for your assistance, Doctor!

If there is a suspicion of recurrence, it is essential to undergo an examination, as recurrence is indeed possible.
It is recommended to return for a follow-up appointment so that the surgeon can monitor the treatment.

After undergoing a high ligation surgery for varicocele in late June, it is understandable to have concerns about the recurrence of symptoms, especially if you are experiencing discomfort in the left testicle again. Varicocele surgery is generally effective in alleviating symptoms associated with enlarged veins in the scrotum, but there are several factors to consider when evaluating your current symptoms.
Firstly, it is important to note that post-operative discomfort can occur for a variety of reasons. While it is possible that you may be experiencing a recurrence of varicocele, it is also possible that your symptoms could be attributed to other factors such as inflammation, nerve irritation, or even muscle strain in the pelvic area. The fact that you do not feel a prominent vein when palpating the spermatic cord is a positive sign, as it suggests that the surgical intervention may have been successful in addressing the major venous issues.
Recurrence of varicocele can happen, but it is not the only explanation for your symptoms. In some cases, the body may take time to heal, and residual discomfort can be a normal part of the recovery process. It is also worth considering that the surgical site may still be healing, which can lead to sensations of tightness or discomfort as the body adjusts.
If you are experiencing persistent pain that does not improve with time or is accompanied by other symptoms such as swelling, changes in testicular size, or any unusual discharge, it is crucial to follow up with your healthcare provider. They may recommend imaging studies, such as a Doppler ultrasound, to assess blood flow in the area and determine if there is any evidence of recurrent varicocele or other complications.
In terms of management, if the discomfort is mild and not accompanied by other concerning symptoms, conservative measures such as rest, ice application, and over-the-counter pain relief may be beneficial. However, if the pain is significant or worsening, it is essential to seek medical advice promptly.
In summary, while the recurrence of varicocele is a possibility, it is not the only explanation for your symptoms. A thorough evaluation by your healthcare provider is necessary to determine the underlying cause of your discomfort and to decide on the appropriate course of action. Regular follow-up appointments are important to monitor your recovery and address any concerns you may have. Your health and well-being are paramount, and open communication with your healthcare team will help ensure that you receive the best possible care.
