Recurrence after varicocele surgery?
I underwent bilateral varicocele surgery at the end of May, and since the end of August, I have been experiencing swelling and pain in the right surgical site and groin area.
However, there are no signs of wound dehiscence.
I would like to know if this could indicate a recurrence.
I haven't engaged in any intense physical activity recently, and my daily routine is normal, but since cohabiting, the frequency of sexual activity has increased.
What could be the issue?

Dr. Sun Hongshun reply Urology
Hello: Just because the wound hasn't opened doesn't mean there isn't an issue.
Internal inflammation or soft tissue strain could be possible.
It is recommended that you consult with the surgeon who performed your operation for an examination.
Until a thorough evaluation is completed, it is advisable to avoid vigorous sexual activity.

After undergoing bilateral varicocele surgery, it is not uncommon to experience some discomfort or pain in the surgical area, especially as you described with the right side of the incision and the groin area. While it is important to monitor these symptoms, they do not necessarily indicate a recurrence of varicocele.
Varicocele surgery, typically performed via laparoscopic or open techniques, aims to alleviate the symptoms associated with enlarged veins in the scrotum. Post-operative discomfort can arise from several factors, including inflammation, nerve irritation, or even the healing process itself. It is also possible that changes in physical activity, such as increased sexual activity, could contribute to the sensation of discomfort, particularly if it involves straining or pressure in the pelvic region.
In terms of recurrence, varicoceles can reoccur, but this is generally more common in cases where the initial surgery was not entirely successful or if there were underlying anatomical issues that were not addressed. Symptoms of a recurrent varicocele may include a noticeable swelling or fullness in the scrotum, a feeling of heaviness, or discomfort that worsens with prolonged standing or physical activity. If you are experiencing persistent pain or swelling, it is advisable to consult your surgeon or a urologist for a thorough evaluation.
To assess whether your symptoms are indicative of a recurrence or simply part of the healing process, your doctor may recommend an ultrasound examination. This imaging study can help visualize the veins in the scrotum and determine if there is any abnormal dilation consistent with a varicocele.
In the meantime, here are some general recommendations to help manage your symptoms:
1. Rest and Recovery: Ensure you are allowing your body adequate time to heal. Avoid heavy lifting or strenuous activities that could exacerbate your symptoms.
2. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, can be helpful for managing discomfort. However, consult your physician before taking any medication.
3. Supportive Garments: Wearing supportive underwear or a jockstrap can help alleviate discomfort by providing support to the scrotal area.
4. Monitor Symptoms: Keep track of any changes in your symptoms, including the intensity and frequency of pain, and report these to your healthcare provider.
5. Follow-Up Appointments: Attend all scheduled follow-up appointments with your surgeon or urologist. These visits are crucial for monitoring your recovery and addressing any concerns you may have.
If your symptoms persist or worsen, or if you notice any new symptoms such as swelling, redness, or fever, it is essential to seek medical attention promptly. Your healthcare provider will be able to provide the best guidance based on your specific situation and medical history.
In summary, while post-operative discomfort is common, it is important to differentiate between normal healing and potential recurrence of varicocele. Regular communication with your healthcare provider and appropriate follow-up care will help ensure a successful recovery.
