Varicocele
Hello Doctor, I had surgery for left-sided varicocele about a year ago.
I started to gradually resume normal activities about a month after the surgery.
Six months post-operation, I occasionally experienced pain at the surgical site due to physical activity, but I thought it was a natural occurrence and didn't pay much attention.
Recently, I have noticed that the varicocele seems to have returned, and I also have developed varicose veins in my left leg, which I did not have before.
When there is engorgement of the penis, the varicocele worsens, and I also experience a feeling of swelling in my left leg, particularly in the areas with varicosities.
However, after a few days of rest, the symptoms tend to subside.
Could you please advise me on how to address this situation? Thank you!

Dr. Xu Weikai reply Urology
Hello, whether there is a recurrence or if further examination is needed.
Generally, varicose veins in the legs are unrelated to varicocele surgery.
If you have concerns, you can visit a nearby medical facility for relevant tests.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
Understanding the recurrence of varicocele and its associated symptoms can be quite concerning, especially after undergoing surgical intervention. Varicocele is a condition characterized by the enlargement of the veins within the scrotum, similar to varicose veins that occur in the legs. It is often associated with discomfort, pain, and potential fertility issues.
In your case, it appears that you underwent surgery for left-sided varicocele about a year ago, and you are now experiencing symptoms that suggest a possible recurrence. The fact that you are noticing pain at the surgical site during physical activity, along with new symptoms such as left leg varicosities and swelling, is significant.
Symptoms and Signs of Recurrence
1. Pain and Discomfort: It is not uncommon to experience some pain post-surgery, especially during physical activities. However, if the pain persists or worsens, it may indicate a recurrence of the varicocele or complications related to the surgery.
2. Visible Varicosities: The appearance of new varicosities in the left leg could be related to the underlying venous insufficiency that may have contributed to the original varicocele. This can occur if the venous valves are not functioning properly, leading to increased pressure in the veins.
3. Swelling: The swelling you describe in your left leg, particularly after activities, could be a sign of venous congestion. This is often exacerbated by prolonged standing or physical exertion.
4. Changes in Symptoms with Activity: The fact that your symptoms improve with rest suggests that they are likely related to venous pressure and blood flow dynamics, which can be affected by physical activity.
Next Steps
Given your symptoms, here are some recommended steps to take:
1. Consultation with a Specialist: It is crucial to follow up with a urologist or a vascular surgeon who specializes in varicocele and venous disorders. They can perform a thorough evaluation, including a physical examination and possibly imaging studies such as Doppler ultrasound, to assess the blood flow in the affected veins.
2. Imaging Studies: A Doppler ultrasound can help visualize the veins in the scrotum and legs, allowing the physician to determine if there is a recurrence of the varicocele or if the new leg symptoms are due to another issue, such as chronic venous insufficiency.
3. Symptom Management: In the meantime, managing your symptoms is important. This may include wearing compression stockings for the leg varicosities, avoiding prolonged standing, and elevating your legs when resting to reduce swelling.
4. Consideration of Further Treatment: If a recurrence of varicocele is confirmed, your doctor may discuss options for further surgical intervention or other treatments to alleviate the symptoms and prevent complications.
5. Monitoring: Keep a record of your symptoms, including when they occur, their severity, and any activities that exacerbate them. This information can be helpful for your healthcare provider in determining the best course of action.
Conclusion
Recurrence of varicocele can occur, and it is essential to address any new symptoms promptly. By consulting with a specialist and undergoing appropriate evaluations, you can gain a clearer understanding of your condition and the best steps to take moving forward. Remember, early intervention can often lead to better outcomes, so don’t hesitate to seek medical advice.
