Please help me, doctors?

Hello, doctor.
I have been suffering from chronic prostatitis for over two years.
I have visited several hospitals, but my condition remains painful and burning.
The doctors always conduct urine tests or ask a few questions and then conclude that I have chronic prostatitis, prescribing anti-inflammatory medications.
Some doctors even go so far as to tell me that I don't need to come back for consultations, saying that there is no effective treatment for this condition.
I have been married for six years and have been wanting to have children, but with my current condition, I am afraid to conceive.
I am truly in pain and distress.
Is it really true that there is no effective treatment? I have seen many hospitals, but nothing has worked.
What should I do? Please, doctors, help me.

Hello, chronic prostatitis can be classified into two types: chronic bacterial prostatitis and chronic non-bacterial prostatitis.
The latter is characterized by an inflammatory response rather than a bacterial infection, and bacteria cannot be isolated from cultures; it may also be caused by hard-to-culture bacteria (such as anaerobes, chlamydia, mycoplasma), fungi, or viruses.

In terms of treatment for chronic bacterial prostatitis, due to the unique tissue characteristics of the prostate, the ideal antibiotics are limited to two classes: Trimethoprim and Quinolones.
The duration of treatment should be at least one month, with some cases requiring three months or even six months, and many patients may experience a recurrence within two years.

Chronic non-bacterial prostatitis is primarily managed through symptomatic treatment and addressing urinary obstruction issues.
Common treatments include alpha-blockers, non-steroidal anti-inflammatory drugs (NSAIDs), and transurethral microwave thermotherapy.
There have also been reports of effectiveness with pollen, zinc, and high doses of vitamins, but it is advisable to avoid spicy and irritating foods (such as chili, coffee, alcohol, etc.).

Chronic prostatitis generally affects semen quality and can impact fertility.

Sincerely, Dr.
Chen Hsiu-Yi, Urologist.

Chronic prostatitis, particularly nonbacterial prostatitis or chronic pelvic pain syndrome (CPPS), can be a challenging condition to manage. It is understandable that after two years of persistent pain and discomfort, you feel desperate for relief and answers. Many patients in similar situations often feel frustrated, especially when they have seen multiple healthcare providers without significant improvement in their symptoms.

Firstly, it is important to acknowledge that chronic prostatitis is a complex condition that does not always respond to conventional treatments. The symptoms can include pelvic pain, urinary urgency, frequency, and discomfort during urination, which can significantly impact your quality of life. Unfortunately, the exact cause of nonbacterial prostatitis is often unclear, which complicates treatment options.

In your case, it seems that you have already undergone various treatments, including anti-inflammatory medications, without satisfactory results. This is not uncommon, as many patients with chronic prostatitis experience a trial-and-error approach to find effective management strategies. Here are some potential avenues to explore further:
1. Multimodal Treatment Approach: Since chronic prostatitis can have multiple contributing factors, a multimodal approach may be beneficial. This can include a combination of medications (such as alpha-blockers, anti-inflammatory drugs, or pain management medications), physical therapy (especially pelvic floor therapy), and lifestyle modifications.

2. Pelvic Floor Physical Therapy: Many patients find relief through pelvic floor physical therapy, which focuses on relaxing and strengthening the pelvic muscles. A trained physical therapist can help you learn techniques to alleviate muscle tension and improve pelvic floor function.

3. Dietary and Lifestyle Changes: Certain foods and beverages can exacerbate prostatitis symptoms. It may be helpful to keep a symptom diary to identify any potential triggers. Common irritants include caffeine, alcohol, spicy foods, and acidic foods. Maintaining a healthy lifestyle with regular exercise, stress management techniques, and adequate hydration can also be beneficial.

4. Alternative Therapies: Some patients report improvement with alternative therapies such as acupuncture, biofeedback, or mindfulness meditation. While these methods may not be universally effective, they can provide additional coping strategies for managing chronic pain.

5. Consideration of Other Conditions: It is essential to rule out other potential causes of your symptoms. Conditions such as interstitial cystitis, pelvic floor dysfunction, or even psychological factors can contribute to chronic pelvic pain. A thorough evaluation by a specialist in pelvic pain or urology may provide further insights.

6. Support Groups and Counseling: Chronic pain can take a toll on mental health. Engaging with support groups or seeking counseling can provide emotional support and coping strategies. Connecting with others who understand your experience can be incredibly validating.

7. Research and Clinical Trials: If conventional treatments have not been effective, you might consider participating in clinical trials. Research is ongoing in the field of prostatitis, and new treatment options may become available.

Regarding your concerns about fertility, chronic prostatitis does not necessarily preclude the possibility of having children. However, it is advisable to discuss your specific situation with a urologist who specializes in male fertility. They can provide guidance on how to manage your condition while considering your desire to conceive.

In conclusion, while chronic prostatitis can be a frustrating and painful condition, there are various strategies to explore beyond standard medication. It may take time and persistence to find the right combination of treatments that work for you. Don't hesitate to seek out specialists who are experienced in managing chronic pelvic pain, as they may offer new perspectives and treatment options. Remember, you are not alone in this journey, and support is available.
