After nasal septum surgery?
I no longer have blood clots in my nose, but why do I still need to take the antihistamine Clarinase two weeks after my septoplasty? If I don't take it, I will experience nasal congestion.
Yesterday, I visited a traditional Chinese medicine doctor due to a cold, and he advised me to be mentally prepared.
The hospital mentioned that the surgical wounds have not fully healed yet and there may still be some swelling, which could be causing my current nasal congestion.
Is it possible that the surgeon only corrected the septum without addressing the hypertrophy of the inferior turbinates?

Dr. Jian Huangqi reply Otolaryngology
You may have only undergone septoplasty and not turbinate reduction surgery, which is why your inferior turbinates may still be enlarged.
Therefore, you will need to continue taking antihistamines.

After undergoing septoplasty, it is not uncommon to experience nasal congestion even two weeks post-surgery. The reasons for this can be multifaceted, and understanding them can help you manage your symptoms more effectively.
Firstly, it is important to recognize that the healing process following nasal surgery can take time. The nasal passages are delicate structures, and after surgery, there may be residual swelling and inflammation that can contribute to feelings of congestion. This is particularly true if the surgery involved any manipulation of the nasal turbinates, which are structures within the nose that help filter and humidify the air we breathe. If the surgeon focused primarily on correcting the septum and did not address any hypertrophy (enlargement) of the turbinates, you may still experience nasal obstruction due to these structures.
Additionally, the use of antihistamines like Clarinase can be beneficial in managing post-operative nasal congestion, especially if you have a history of allergies. Antihistamines work by blocking the action of histamine, a substance in the body that can cause allergic symptoms such as nasal congestion, sneezing, and itching. However, they can also lead to dryness in the nasal passages, which may not be ideal for someone recovering from surgery. It is a balancing act between alleviating congestion and managing dryness.
Your experience of needing to take antihistamines to relieve nasal congestion suggests that there may be an allergic component to your symptoms. Allergies can exacerbate nasal congestion, particularly in the post-operative period when the nasal mucosa is still healing. It is also possible that environmental factors, such as dust or pollen, could be contributing to your symptoms.
In terms of your concerns about the surgical technique, it is essential to have a clear understanding of what was performed during your septoplasty. If the surgeon did not address any underlying issues with the turbinates, it could explain why you are still experiencing nasal congestion. It may be beneficial to follow up with your ENT specialist to discuss your ongoing symptoms and whether additional treatment options, such as turbinate reduction, might be appropriate.
In summary, post-septoplasty nasal congestion can be attributed to several factors, including residual swelling, potential allergic reactions, and the surgical focus on the septum rather than the turbinates. While antihistamines can help manage symptoms, it is crucial to communicate with your healthcare provider about your ongoing issues to ensure a comprehensive approach to your recovery. They may recommend additional treatments or adjustments to your current regimen to help you achieve better nasal airflow and comfort.
