Redness and swelling

Hello doctor, recently I have noticed a red, swollen lump about two to three centimeters above the base of my penis (in the lower abdomen area? I'm not sure how to describe it...) Anyway, it's around the pubic hair area and it hurts a little when I press on it.
The texture feels different from normal skin (slightly hard?).
I apologize for my poor description.
What could this symptom possibly indicate? Thank you in advance.
(I have only had two sexual partners, which are my left hand and right hand; is there a possibility of a sexually transmitted infection?)

Xin Jing Cha, 10~19 year old female. Ask Date: 2005/09/12

Dr. Xu Weikai reply Urology

New Officer: Hello, I believe it is a localized infection.
After a while, it will soften, and a fluid resembling pus will start to drain out.
At that time, you can gently apply pressure to expel the fluid and then apply some ointment.
For now, do not manipulate it.

When experiencing red swelling in the urogenital area, particularly near the base of the penis, it is essential to consider various potential causes. The symptoms you describe—a red, swollen area that is slightly painful to the touch and has a different texture than normal skin—could indicate several conditions, some of which may require medical attention.

1. Infection: One of the most common causes of localized swelling and redness in the genital area is an infection. This could be a sexually transmitted infection (STI) such as herpes, gonorrhea, or chlamydia, especially if you have had unprotected sexual contact. However, infections can also be non-sexually transmitted, such as a fungal infection (like candidiasis) or a bacterial infection (like folliculitis).

2. Inflammation: Conditions such as balanitis (inflammation of the glans penis) or posthitis (inflammation of the foreskin) can also lead to redness and swelling. These conditions can be caused by irritants, poor hygiene, or infections.

3. Dermatological Issues: Skin conditions such as dermatitis, psoriasis, or eczema can manifest as red, swollen patches in the genital area. These conditions may not necessarily be related to sexual activity and can occur due to various triggers, including allergens or irritants.

4. Trauma: If you have recently engaged in activities that may have caused trauma to the area (such as vigorous sexual activity or injury), this could lead to localized swelling and pain.

5. Cysts or Abscesses: Sometimes, sebaceous cysts or abscesses can form in the genital area, leading to localized swelling and tenderness. These may require drainage or other medical interventions.

Given your concern about potential sexually transmitted infections, it is crucial to consider the possibility of STIs, even if you believe your sexual partners have been limited. Many STIs can be asymptomatic, meaning that individuals may not show symptoms but can still transmit infections.

Recommendations:
- Seek Medical Attention: It is advisable to consult a healthcare provider for a thorough examination. They may perform tests, including swabs or blood tests, to determine the underlying cause of your symptoms.

- Avoid Self-Diagnosis: While it is helpful to be informed about potential causes, self-diagnosing can lead to unnecessary anxiety and may delay appropriate treatment.

- Practice Safe Sex: If you are sexually active, consider using protection to reduce the risk of STIs. Open communication with partners about sexual health is also essential.

- Monitor Symptoms: Keep track of any changes in your symptoms, such as increased pain, discharge, or fever, and report these to your healthcare provider.

In conclusion, while there are several potential causes for the symptoms you are experiencing, a medical evaluation is necessary to determine the exact cause and appropriate treatment. Early intervention can help prevent complications and ensure better health outcomes.

Your symptoms are non-specific, meaning there could be many possible causes. What I mean is that if there has been any risky sexual behavior, sexually transmitted infections cannot be ruled out. It is advisable to see a urologist for blood tests and follow-up.
