Difficulty breathing?

Hello Doctor: Sometimes I feel like my breathing is not smooth, and at times I make strange noises from my throat.
Although I want to control it and not make those noises, I feel uncomfortable if I don't.
I have previously been examined at a large hospital, but the doctors said everything was normal.
I still don't know what is going on, so I would like to ask for your advice.

Xie Yi Xiao Yao, 10~19 year old female. Ask Date: 2005/10/16

Dr. Qu Changke reply Pulmonology

Hello, Evil Winged Fairy: May I ask what tests you have undergone? You might have issues with airway hyperreactivity.
A pulmonary function test could be helpful.
Wishing you good health, Dr.
Chiu.

Breathing difficulties can be a perplexing and distressing experience, especially when medical evaluations yield normal results. Your description of experiencing irregular breathing and producing unusual sounds from your throat suggests a multifaceted issue that may involve both physiological and psychological components.
Firstly, it’s essential to understand that breathing is a complex process regulated by the brain, lungs, and various muscles. When you feel that your breathing is not smooth, it may be due to several factors, including anxiety, environmental triggers, or underlying health conditions.

1. Anxiety and Stress: One of the most common causes of perceived breathing difficulties is anxiety. When you are anxious, your body enters a state of heightened alertness, which can lead to hyperventilation or a feeling of breathlessness. This can create a cycle where the fear of not being able to breathe properly exacerbates the anxiety, leading to more pronounced symptoms. It’s important to practice relaxation techniques such as deep breathing exercises, mindfulness, or yoga to help manage anxiety levels.

2. Environmental Factors: If your breathing difficulties occur in specific settings, such as a classroom or a crowded area, it could be related to environmental factors. Poor air quality, allergens, or even the psychological pressure of being in a particular environment can contribute to feelings of breathlessness. Consider evaluating the air quality in your surroundings and whether there are any potential allergens that could be affecting you.

3. Physical Health Considerations: Although previous medical evaluations have indicated normal results, it may be worthwhile to revisit your healthcare provider for a comprehensive assessment. Conditions such as asthma, chronic obstructive pulmonary disease (COPD), or even gastroesophageal reflux disease (GERD) can manifest as breathing difficulties. Additionally, if you have a history of allergies or nasal congestion, it could lead to breathing issues, particularly if there is a blockage in the nasal passages.

4. Muscle Tension: Sometimes, the muscles involved in breathing can become tense due to stress or poor posture. This tension can restrict your ability to take deep breaths, leading to a sensation of breathlessness. Engaging in regular physical activity, stretching, and maintaining good posture can help alleviate this tension.

5. Vocal Cord Dysfunction: The unusual sounds you describe may also indicate a condition known as vocal cord dysfunction (VCD), where the vocal cords do not open properly during inhalation, leading to a feeling of tightness in the throat and abnormal sounds. This condition can be triggered by stress, exercise, or irritants in the environment. A speech therapist or an ear, nose, and throat (ENT) specialist can provide guidance on managing VCD.

In conclusion, while your previous medical evaluations have not revealed any significant issues, the experience of breathing difficulties can stem from a combination of psychological and physiological factors. It is crucial to address both aspects to find relief. Consider consulting with a healthcare provider who specializes in respiratory issues or a mental health professional to explore the potential interplay between anxiety and your breathing difficulties. Regular exercise, stress management techniques, and possibly further evaluations may help you better understand and manage your symptoms. Remember, you are not alone in this experience, and seeking help is a positive step towards improving your quality of life.
