Anal itching

Hello, two weeks ago I noticed itching around the anal area with about 4-5 small raised bumps.
They itch more when I scratch them, and I haven't had any sexual experiences this month.
I've been very careful with hygiene.
What could this condition be, and which specialist should I see? Thank you.

Hello: In response to your question, anal itching can sometimes be caused by excessive cleaning, leading to localized skin inflammation.
In addition to recommending an examination by a dermatologist, it is generally advised to avoid scratching and using soap for cleaning.
As for the protruding lesions, there are two possibilities: external hemorrhoids or condyloma acuminatum (commonly known as genital warts).
It is essential to consult a colorectal surgeon for an accurate diagnosis.

More Info

Anal itching, also known as pruritus ani, is a common condition that can be caused by a variety of factors. Understanding the potential causes, symptoms, and when to seek medical help is crucial for effective management.

Causes of Anal Itching
1. Skin Irritation: This can occur due to excessive moisture, poor hygiene, or irritation from soaps, toilet paper, or other products. Conditions like eczema or psoriasis can also contribute to skin irritation in the anal area.

2. Infections: Fungal infections, such as candidiasis, or bacterial infections can lead to itching. Additionally, sexually transmitted infections (STIs) can cause anal itching, although you mentioned not having sexual experiences recently.

3. Hemorrhoids: Swollen veins in the rectal area can cause discomfort and itching. Hemorrhoids can be internal or external and may be accompanied by bleeding or a lump near the anus.

4. Pinworms: Particularly in children, pinworm infections can cause intense itching around the anus, especially at night. Adult infections are less common but can occur.

5. Dietary Factors: Certain foods, such as spicy foods, caffeine, and alcohol, can irritate the anal area and lead to itching.

6. Skin Conditions: Conditions like dermatitis, fungal infections, or even certain sexually transmitted infections can manifest as itching or irritation around the anus.

Symptoms to Look For
- Itching: The primary symptom is itching around the anal area, which can sometimes be severe.

- Rash or Bumps: You mentioned noticing small bumps; this could indicate a variety of conditions, including infections or skin irritations.

- Discomfort or Pain: Some individuals may experience pain or discomfort, especially when sitting or during bowel movements.

- Bleeding: If you notice blood during wiping or in your stool, this is a significant symptom that requires immediate medical attention.

When to Seek Help
You should consider seeking medical help if:
- The itching persists for more than a few days despite home care measures.

- You notice any bleeding, discharge, or significant pain.

- The bumps or rash worsen or do not improve with over-the-counter treatments.

- You have any concerns about sexually transmitted infections, even if you have not had recent sexual experiences.

Which Specialist to See
For anal itching, you would typically start by consulting a primary care physician or a dermatologist. If the issue seems to be related to gastrointestinal problems, a gastroenterologist may be appropriate. If you suspect an infection or a sexually transmitted disease, a visit to an infectious disease specialist or a urologist may be warranted.

Conclusion
Anal itching can be uncomfortable and distressing, but understanding its potential causes and symptoms can help you take appropriate action. Maintaining good hygiene, avoiding irritants, and monitoring your symptoms are essential steps. If the condition persists or worsens, do not hesitate to seek professional medical advice to ensure proper diagnosis and treatment. Remember, it’s always better to address health concerns sooner rather than later to prevent complications.
