Low back pain
Hello doctor, I have been experiencing a dull pain in my left lower back since a year ago when I stand for a long time.
Gradually, the pain occurs even after standing for a short while.
Initially, resting while sitting would alleviate the pain, but recently, I have started to feel pain even while sitting, and it takes a long time to feel better.
Could you please tell me what might be the cause of this, and what methods or medications could help improve my condition?

Dear Navy 3839 user,
One year ago, you started experiencing a dull pain in your left lower back after standing for long periods.
Gradually, the pain has worsened to the point where it occurs even after standing for a short time.
Initially, resting while sitting would alleviate the pain, but recently, you have found that sitting also causes pain, requiring extended periods of rest for relief.
This may be indicative of "lumbar degenerative syndrome," which is a common condition among office workers due to prolonged standing and the cumulative pressure on the lumbar region, compounded by poor sitting posture over time.
This can lead to lumbar degenerative pain and may even radiate pain to the lower back and thighs.
The most common cause of lower back pain is often attributed to "myofascial pain syndrome," although some cases may involve lumbar osteophytes (bone spurs), lumbar spondylolisthesis, or inflammation of the paraspinal muscles.
Most of these conditions are unrelated to kidney function.
If you are concerned about kidney health, a simple kidney function test (BUN and creatinine) or a urinalysis can provide initial insights into kidney function.
I recommend consulting a qualified Traditional Chinese Medicine (TCM) practitioner for acupuncture treatment or medication, which may yield positive results.
Regarding medication, I personally tend to use those that promote blood circulation and relieve muscle tension.
Sometimes, if there is a concern about kidney deficiency, tonifying kidney medications may be prescribed.
It is advisable to consult a qualified TCM practitioner to determine the most suitable medication for your constitution, ensuring it is appropriate and free of side effects.
Wishing you good health,
Dr.

More Info
Chronic lower back pain is a common issue that affects many individuals, and it can arise from a variety of underlying causes. Based on your description, it seems that your pain has progressively worsened over the past year, transitioning from discomfort while standing to pain even when sitting. This progression is concerning and warrants a thorough evaluation.
Possible Causes of Chronic Lower Back Pain
1. Muscle Strain or Ligament Sprain: Overuse or improper lifting can lead to muscle strains or ligament sprains. These injuries can cause localized pain that may worsen with prolonged standing or sitting.
2. Degenerative Disc Disease: As we age, the intervertebral discs can lose hydration and elasticity, leading to pain. This condition can cause discomfort during activities that put pressure on the spine.
3. Herniated Disc: A herniated disc occurs when the soft material inside the disc bulges out, potentially pressing on nearby nerves. This can lead to pain that radiates down the leg, known as sciatica.
4. Spinal Stenosis: This condition involves the narrowing of the spinal canal, which can compress the spinal cord and nerves, leading to pain, numbness, or weakness in the legs.
5. Sacroiliac Joint Dysfunction: The sacroiliac joint connects the spine to the pelvis. Dysfunction in this joint can cause pain in the lower back and buttocks, often exacerbated by prolonged sitting or standing.
6. Osteoarthritis: Wear and tear on the joints can lead to osteoarthritis, which may cause pain and stiffness in the lower back.
7. Other Conditions: Conditions such as scoliosis, ankylosing spondylitis, or infections can also contribute to chronic lower back pain.
Recommended Diagnostic Approaches
To determine the exact cause of your pain, a comprehensive evaluation is necessary. This may include:
- Physical Examination: A healthcare provider will assess your posture, range of motion, and any areas of tenderness.
- Imaging Studies: X-rays, MRI, or CT scans can provide detailed images of the spine and help identify any structural issues, such as herniated discs or spinal stenosis.
- Blood Tests: In some cases, blood tests may be necessary to rule out infections or inflammatory conditions.
Treatment Options
Once a diagnosis is established, treatment can be tailored to your specific condition. Common treatment options include:
1. Physical Therapy: A physical therapist can design a personalized exercise program to strengthen the muscles supporting your back, improve flexibility, and promote better posture.
2. Medications: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain. In some cases, prescription medications, including muscle relaxants or anti-inflammatory drugs, may be necessary.
3. Heat and Cold Therapy: Applying heat or cold packs can provide relief from pain and inflammation.
4. Lifestyle Modifications: Maintaining a healthy weight, practicing good posture, and incorporating regular physical activity can significantly improve back health.
5. Injections: In cases of severe pain, corticosteroid injections may be used to reduce inflammation around the affected nerves.
6. Surgery: If conservative treatments fail and the pain significantly impacts your quality of life, surgical options may be considered, particularly for conditions like herniated discs or spinal stenosis.
Conclusion
Chronic lower back pain can be multifactorial, and understanding the underlying cause is crucial for effective treatment. It is essential to consult with a healthcare professional who can guide you through the diagnostic process and recommend appropriate treatment options. Early intervention can lead to better outcomes and a return to your daily activities with reduced pain. Remember, managing chronic pain often requires a comprehensive approach that includes physical, medical, and lifestyle strategies.
