The sad situation of hyperactive children and their parents?

Dear Doctor,
This issue has troubled me for a long time.
My son, who is 6 years old this year, was diagnosed with Attention Deficit Hyperactivity Disorder (ADHD) two years ago at Puxin Chang Hospital.
The doctor told me to wait for notification regarding treatment, but after a while with no news, I had to inquire again and learned that Changhua Christian Hospital offers this treatment.
We switched to Changhua Christian Hospital, and the treatment process went relatively smoothly.
However, unexpectedly, just after 2 or 3 months, the therapist got married and left, which was unfortunate.
The treatment was interrupted.
We then hurried back to Chang Hospital to register for treatment again.
After a long wait, we finally received notification and thought everything would go smoothly from then on.
But to my surprise, after just 2 months of treatment, the therapist informed us that due to a lack of qualified staff, treatment could not continue.
I truly cannot believe this.
Is the medical care in Changhua really this inadequate? Why is it that the northern regions and Taichung can provide such good services? Are we, the residents of Changhua County, considered second-class citizens? I am just a person who barely graduated from junior high school, and my knowledge is limited.
Where should I seek assistance?

Dr. Zhang Wenwang reply Pediatrics

Changhua Hospital has newly established a Child Psychiatry department on Wednesday and Friday afternoons.
Resources such as physicians and therapists are gradually increasing, which may be able to assist you.

The challenges faced by parents of children with Attention Deficit Hyperactivity Disorder (ADHD) in Taiwan's healthcare system can be quite significant, as highlighted by your experience. It is essential to understand that ADHD is a neurodevelopmental disorder that requires a comprehensive approach to diagnosis and treatment, which can often be hindered by systemic issues within the healthcare framework.

Firstly, the diagnostic process for ADHD typically involves a thorough evaluation by a qualified healthcare professional, which may include pediatricians, child psychiatrists, or psychologists. Unfortunately, as you have experienced, there can be significant delays in receiving treatment once a diagnosis is made. This is often due to a shortage of qualified professionals, particularly in certain regions like Changhua, where you reside. The lack of available specialists can lead to long waiting times for appointments and treatment, which can be frustrating for parents who are eager to help their children.

Moreover, the continuity of care is crucial for managing ADHD effectively. The abrupt changes in treatment providers, such as therapists leaving or being unavailable, can disrupt the therapeutic process. This inconsistency can lead to setbacks in the child's progress, as building a rapport with a therapist is essential for effective treatment. Parents may feel helpless in these situations, as they are often left to navigate the system without adequate support or guidance.

Another challenge is the perception of ADHD within the community and the healthcare system. There can be stigma associated with ADHD, leading to misunderstandings about the disorder. Some parents may feel that their concerns are not taken seriously, especially if their child does not exhibit the more overt symptoms of hyperactivity. This can result in a lack of urgency in addressing the child's needs, further complicating the situation.

In terms of resources, parents often find themselves at a disadvantage, particularly if they lack knowledge about the healthcare system or available support services. Many parents may not know where to turn for help or how to advocate for their child's needs effectively. This can lead to feelings of isolation and frustration, as they struggle to find appropriate care for their children.

To address these challenges, it is essential for parents to seek out community resources and support networks. Connecting with local ADHD support groups can provide valuable information and emotional support. Additionally, parents should not hesitate to reach out to their healthcare providers for guidance on navigating the system and accessing the necessary services.

In your case, it may be beneficial to explore other healthcare facilities or specialists in nearby regions if the current options in Changhua are limited. Some hospitals may have pediatric mental health departments that accept adult patients or have more resources available. It is also worth considering telehealth options, which have become more prevalent and can provide access to specialists who may not be available locally.

In conclusion, while the challenges faced by parents of children with ADHD in Taiwan's healthcare system can be daunting, there are steps that can be taken to improve the situation. By advocating for their child's needs, seeking out community resources, and exploring alternative care options, parents can work towards ensuring that their children receive the support and treatment they need to thrive.
