Masturbation
Hello, doctor.
I have seen some of your previous answers and I would like to ask a question.
I have a long-term habit of masturbation (since I was 18 years old), and I believe this is the reason for my current erectile difficulties.
Sometimes I can achieve an erection while watching adult films, but if I don't continue stimulation, it quickly fades away.
I have followed your advice to concentrate (without any manual stimulation), but out of ten attempts, only one or two are successful.
Additionally, I have never succeeded while standing; I usually only succeed when sitting or lying down.
Sometimes, when I look at adult images, I experience a slight erection that quickly subsides.
I do not smoke or drink alcohol, and my daily routine is normal.
I have been practicing abstinence since January.
I regret my excessive masturbation in the past, which has led to my current situation where I feel desire but lack the ability.
I have a girl I like, and I am worried this might create a barrier between us (I have never had sexual intercourse).
I am unsure if this is erectile dysfunction or venous leakage.
I am under a lot of stress.
Please give me some advice, thank you, doctor.
Mr. Cheng, 20~29 year old female. Ask Date: 2006/02/15
Dr. Xu Weikai reply Urology
Mr.
Cheng: Hello, I believe that at your age, you should not be experiencing organic erectile dysfunction, especially since you have previously engaged in DIY activities.
Typically, these issues are related to psychological factors, often influenced by misleading advertisements and radio discussions that claim DIY can negatively impact physical and mental development.
Here’s a statistic: according to studies, over half of men have had DIY experiences.
This indicates that DIY does not cause erectile dysfunction.
In fact, some specialists in sexual dysfunction even encourage DIY to stimulate local neurovascular responses and enhance sexual function.
I think you are overly concerned about sexual performance (perhaps due to the overwhelming media coverage), which is quite natural.
Research shows that the success rate for first sexual encounters is less than 20%.
This illustrates that sexual activity requires experience, skill, and learning.
More importantly, both partners need to be familiar with each other and cooperate to achieve satisfaction.
If you are truly worried, consider visiting a major hospital for a comprehensive evaluation, which will provide a more accurate diagnosis.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Masturbation is a common and natural behavior that many individuals engage in throughout their lives. Concerns about its impact on erectile function, particularly in the context of excessive or compulsive masturbation, are not uncommon. It’s important to understand the physiological and psychological aspects of sexual function to address your concerns effectively.
Firstly, it’s essential to clarify that masturbation itself, when practiced in moderation, does not inherently lead to erectile dysfunction (ED). However, excessive masturbation can sometimes lead to psychological factors that may affect sexual performance. For instance, if an individual has developed a habit of relying on specific stimuli (such as pornography) to achieve arousal, they may find it challenging to achieve the same level of arousal in different contexts, such as with a partner. This phenomenon is often referred to as "performance anxiety," where the pressure to perform can lead to difficulties in achieving or maintaining an erection.
Your experience of being able to achieve an erection while watching adult content but struggling to maintain it without continued stimulation is not uncommon. This can be attributed to a few factors:
1. Desensitization: Regular exposure to high levels of sexual stimuli, such as pornography, can lead to desensitization. This means that over time, you may require more intense or specific stimuli to achieve the same level of arousal, which can make it difficult to respond to a partner in a more intimate setting.
2. Anxiety and Stress: The pressure to perform, especially with someone you are interested in, can create anxiety that interferes with your ability to relax and enjoy the moment. This anxiety can lead to a cycle of worry that further exacerbates erectile difficulties.
3. Physical Factors: While you mentioned that you do not smoke or drink and maintain a regular schedule, it’s still important to consider other physical factors that could contribute to erectile issues. Conditions such as hormonal imbalances, vascular issues, or even certain medications can impact erectile function.
4. Psychological Factors: Your feelings of regret about past behaviors and concerns about your sexual performance can create a negative feedback loop. This can lead to increased anxiety and stress, which can further impair erectile function.
To address your concerns, consider the following steps:
- Limit Pornography Use: Reducing or eliminating pornography consumption can help reset your arousal responses and allow you to engage more fully with a partner.
- Focus on Relaxation: Practice relaxation techniques, such as deep breathing or mindfulness, to help reduce performance anxiety. This can help you feel more at ease during intimate moments.
- Communicate with Your Partner: Open communication with your partner about your feelings and concerns can foster intimacy and understanding, which may alleviate some of the pressure you feel.
- Seek Professional Help: If your concerns persist, consider speaking with a healthcare professional or a therapist who specializes in sexual health. They can provide tailored advice and support to help you navigate these challenges.
- Physical Health Check: If you have ongoing concerns about erectile function, it may be worthwhile to consult a physician for a thorough evaluation to rule out any underlying medical conditions.
In summary, while excessive masturbation can lead to psychological barriers that may affect erectile function, it is not the sole cause of erectile dysfunction. By addressing the psychological aspects, reducing reliance on specific stimuli, and seeking professional guidance if needed, you can work towards improving your sexual health and confidence. Remember, sexual health is a complex interplay of physical, psychological, and relational factors, and it’s important to approach it holistically.
