A strange sensation?
Hello, doctor: During my university years, as the academic pressure became less intense, I found that my sleep duration increased significantly.
I could sleep until 11 AM if I had no classes, and even take a nap in the afternoon without any issues.
However, I gradually noticed that the outer side of my head would develop a feeling of pressure if I slept on the same side for less than five minutes.
This sensation is located on the outer scalp.
I do not experience headaches or dizziness; rather, the feeling of pressure has become more pronounced than before.
If I switch sides, the sensation disappears, but it reappears on the other side within five minutes as well.
Additionally, I now experience soreness in both shoulders, which contributes to discomfort in my head.
When I feel uncomfortable, moving my head or shoulders produces a faint "crackling" sound in my neck.
In recent years, I have also noticed a slight decline in my attention and concentration.
I would like to ask what might be causing this.
Could it be due to frequently staring at the television or computer screen? I spend about seven to eight hours a day on the computer and watching TV.

Dr. Ding Xianwei reply Neurosurgery
Hello: You may be experiencing symptoms due to overuse of your cervical spine, possibly from staring at the television or computer screen.
Therefore, it is recommended that you avoid prolonged standing or sitting, and refrain from maintaining a fixed posture for too long, as this may help alleviate your symptoms.
If you have further questions, please consult a neurosurgery outpatient clinic.
Sincerely, Dr.
Ding Hsien-Wei, Neurosurgery.

Understanding unusual head pressure can be a complex issue, especially when it involves symptoms that seem to be linked to lifestyle factors such as prolonged periods of sleep, screen time, and physical tension. Based on your description, it appears that you are experiencing a sensation of pressure on the outer sides of your head, particularly when lying on one side for an extended period. This sensation is accompanied by shoulder discomfort and a decrease in attention and concentration.
Firstly, the pressure sensation you are experiencing could be attributed to several factors. One possibility is that it may be related to muscle tension in the neck and shoulders. When you sleep on one side for too long, it can lead to muscle strain and tension, which may manifest as a feeling of pressure in the head. The "crackling" sound you hear when moving your neck could indicate joint or ligament movement, which is often benign but can be exacerbated by muscle tightness or misalignment.
Additionally, your reported decrease in attention and concentration could be linked to several factors, including fatigue, poor sleep quality, or even the effects of prolonged screen time. Extended exposure to screens can lead to digital eye strain, which may cause discomfort, headaches, and difficulty focusing. This is often referred to as computer vision syndrome, and it can be mitigated by taking regular breaks, practicing the 20-20-20 rule (every 20 minutes, look at something 20 feet away for 20 seconds), and ensuring proper ergonomics while using screens.
Moreover, the sensation of pressure could also be a sign of tension-type headaches, which are often triggered by stress, poor posture, or muscle tension. These headaches typically do not present with the severe symptoms associated with migraines or other more serious conditions, but they can still be quite uncomfortable and affect your quality of life.
It's also worth considering the impact of your sleep patterns. While longer sleep durations can be beneficial, irregular sleep schedules or oversleeping can lead to feelings of grogginess and decreased cognitive function. Establishing a consistent sleep routine and ensuring that you are getting restorative sleep can help improve your overall well-being.
Given your symptoms, it would be advisable to consult with a healthcare professional, such as a neurologist or a primary care physician, who can conduct a thorough evaluation. They may recommend imaging studies, such as an MRI or CT scan, to rule out any structural issues or other underlying conditions. Additionally, they may suggest physical therapy to address muscle tension and improve posture, as well as lifestyle modifications to enhance your overall health.
In summary, the sensations you are experiencing could be multifactorial, involving muscle tension, sleep patterns, and the effects of prolonged screen time. Addressing these factors through lifestyle changes, ergonomic adjustments, and possibly medical intervention can help alleviate your symptoms and improve your quality of life. Remember, it’s essential to listen to your body and seek professional advice if your symptoms persist or worsen.
