Concerns About Using Proscar for Hair Loss: A Urology Perspective - Urology

Issues related to Proscar (finasteride)?

Hello Dr.
Lin,
I am 29 years old and have been experiencing hair loss for the past three years.
I was initially taking Finasteride (Rogaine), but due to the high cost, I switched to Proscar, which contains the same active ingredient.
However, Finasteride is 1mg while Proscar is 5mg.
I am taking Proscar 5mg every four days (without splitting the tablet).
I understand that Proscar is used to treat benign prostatic hyperplasia.

I have two questions:
1.
Will my method of taking Proscar 5mg every four days (without splitting) have any effects on my body, including sexual function, fertility issues (as I am not yet married), or any other concerns?
2.
Given my age, is it appropriate for me to take Proscar 5mg?
Thank you for your response.

Dr. Lin Hexing reply Urology

Hello, Xiao B: I wonder how effective you find the use of Tadalafil.
If it is effective, you can certainly consider substituting it with Bosentan to save costs.
However, it is recommended to take 4 to 5 doses per week, with one dose each day to maintain effectiveness, as Bosentan is not a long-acting medication.
Taking it every four days will not achieve the necessary therapeutic drug concentration.
As for whether it will affect sexual function, you will know during your treatment.
If there is an effect, it will be noticeable, but it should improve after discontinuation.
If there is no impact on sexual function, then it is not a concern.
With a daily dose of 5 mg of Bosentan, the likelihood of affecting sexual function is about 15%, so it does not necessarily impact sexual function.

More Info

When it comes to hair loss treatments, particularly with medications like Proscar (finasteride), there are several important considerations to keep in mind, especially from a urology perspective.
1. Dosage and Administration: You mentioned that you are taking Proscar (5mg) every four days instead of the standard daily dosage of 1mg typically prescribed for hair loss. While Proscar and Propecia (the 1mg formulation) contain the same active ingredient, finasteride, the dosing frequency and amount can significantly impact the effectiveness and side effects. Taking a higher dose less frequently may not provide the same benefits as taking a lower dose daily. The half-life of finasteride is about 6-8 hours, meaning that the drug's effectiveness diminishes over time. Therefore, taking it every four days may lead to suboptimal levels of the medication in your system, potentially reducing its effectiveness in preventing hair loss.

2. Potential Side Effects: Finasteride is known to have side effects, particularly concerning sexual function. These can include decreased libido, erectile dysfunction, and ejaculation disorders. While these side effects are relatively uncommon, they can occur. Since you are concerned about future fertility and sexual health, it is crucial to discuss these potential risks with your healthcare provider. They can provide personalized advice based on your medical history and current health status.

3. Age Considerations: At 29 years old, you are within the age range where androgenetic alopecia (male pattern baldness) can begin to manifest. Finasteride is generally considered safe for use in younger adults, but it is essential to weigh the benefits against the risks. If you have concerns about the long-term effects of taking a medication like Proscar, especially regarding sexual function and fertility, it would be wise to consult with a urologist or a dermatologist specializing in hair loss. They can help you understand the implications of long-term use and explore alternative treatments if necessary.

4. Monitoring and Follow-Up: Regular follow-up appointments with your healthcare provider are essential while on finasteride. They can monitor for any side effects and assess the effectiveness of the treatment. If you experience any adverse effects, it is crucial to report them immediately. Additionally, if you are considering starting a family in the future, discussing your plans with your doctor can help you make informed decisions about your treatment options.

5. Alternative Treatments: If the cost of Propecia is a concern, there may be other treatment options available, including topical treatments like minoxidil, which can be used in conjunction with oral medications. Lifestyle changes, such as a balanced diet, stress management, and proper hair care, can also play a role in managing hair loss.

In conclusion, while Proscar can be an effective treatment for hair loss, it is essential to use it correctly and be aware of potential side effects. Given your concerns about sexual function and fertility, I strongly recommend discussing your treatment plan with a healthcare professional who can provide tailored advice and support.
