Chronic widespread pain and stiffness?

Hello Dr.
Yeh,
I would like to ask you some questions.
My mother has been experiencing widespread body aches for a long time, and it has progressively worsened.
Now, she is suffering from both pain and stiffness.
For example, her ten finger joints are stiff and painful, making it difficult for her to exert force even when using scissors.
The joint where her arm meets her shoulder hurts so much that she can hardly lift it or move it backward.
Her elbow joints, which have not been injured, hurt just by touching them.
Both of her legs are sore, and she often struggles to get on the back of a motorcycle.
A few days ago, my mother felt unwell all over and had trouble sleeping for several days.
I tried to help her by using essential oils for a full-body massage, hoping it would help her sleep better at night.
Surprisingly, while she felt comfortable afterward, the pain intensified to an unbearable level.
She had to take pain relief and anti-inflammatory medication in the middle of the night to find some relief.
My mother has previously seen an orthopedic doctor and underwent bone and density tests, all of which came back normal.
The doctor mentioned that her bone age appears to be in the 70s, even though she is currently 55 years old.
Whenever she experiences pain and visits a doctor, she is prescribed muscle relaxants and anti-inflammatory medications, but it seems like these only address the symptoms rather than the underlying issue.
She has also attended physical therapy a few times, but she felt that the simple exercises did not lead to any improvement, so she stopped going.
Given the above situation, I would like to ask what might be wrong with my mother's body.
Could it be nerve inflammation? Should she see a neurologist or a neurosurgeon? Are there any further tests needed to investigate the cause of her condition? Besides taking pain relief and anti-inflammatory medications, are there any other more effective methods? Is it possible for her to achieve complete recovery after treatment?
I would greatly appreciate your guidance on the direction for her medical care.
Thank you very much, and I wish you all the best.

It appears to be arthritis.
It is recommended to see a rheumatologist to check for rheumatoid arthritis or other underlying causes.

Chronic pain and stiffness can be complex and multifactorial, often requiring a comprehensive approach to diagnosis and management. Based on your description, it seems that your mother is experiencing widespread pain and stiffness, particularly in her joints, which has progressively worsened over time. This situation can be quite distressing, not only for her but also for family members who wish to help.

Firstly, it’s important to recognize that chronic pain can arise from various underlying conditions, including but not limited to arthritis, fibromyalgia, myofascial pain syndrome, or even autoimmune disorders. Given that your mother has had normal bone density tests and has been prescribed muscle relaxants and anti-inflammatory medications without significant improvement, it may be beneficial to explore other potential causes of her symptoms.

1. Rheumatological Evaluation: Since she is experiencing joint pain and stiffness, a consultation with a rheumatologist could be valuable. Rheumatologists specialize in autoimmune and inflammatory conditions that affect the joints and soft tissues. Conditions such as rheumatoid arthritis, lupus, or psoriatic arthritis could be considered, especially if there are any accompanying symptoms like fatigue, skin changes, or morning stiffness.

2. Neurological Assessment: If there is a suspicion of nerve involvement, a neurologist may be able to provide insights into whether there is any neuropathic pain component. Conditions like carpal tunnel syndrome or peripheral neuropathy could contribute to her symptoms, particularly in her hands and arms.

3. Physical Therapy: While you mentioned that previous physical therapy sessions did not yield significant results, it may be worthwhile to seek a specialized physical therapist who can tailor a program specifically for her needs. A therapist experienced in chronic pain management can introduce gentle stretching, strengthening exercises, and modalities such as heat or ultrasound therapy, which may help alleviate some discomfort.

4. Pain Management: A pain management specialist can offer additional strategies, including interventional procedures such as nerve blocks or injections, which may provide more substantial relief than oral medications alone. They can also discuss the potential benefits of alternative therapies, such as acupuncture or chiropractic care.

5. Lifestyle Modifications: Encouraging your mother to engage in low-impact activities, such as swimming or walking, can help maintain joint function and reduce stiffness. Additionally, weight management can play a crucial role in alleviating stress on the joints. Nutritional support, including anti-inflammatory diets rich in omega-3 fatty acids, may also be beneficial.

6. Psychological Support: Chronic pain can often lead to emotional distress, including anxiety and depression. Cognitive-behavioral therapy (CBT) or other forms of psychological support can help her develop coping strategies and improve her overall quality of life.

7. Further Investigations: If her symptoms persist despite these interventions, further imaging studies such as MRI or ultrasound may be warranted to assess for any underlying joint or soft tissue abnormalities that were not visible on previous X-rays.

In conclusion, while it is understandable to seek immediate relief through medications, a more holistic approach that includes multidisciplinary care may provide better long-term outcomes. It is essential to work closely with healthcare providers to identify the root cause of her symptoms and develop a comprehensive treatment plan tailored to her specific needs. With the right support and interventions, there is potential for significant improvement in her condition.
