Masturbation issues

Hello, Dr.
Hsu:
Actually, I have had this issue for 2 to 3 years and have been hesitant to seek medical help.
I came across this consultation webpage and would like to ask for your advice, thank you.
Here’s the situation: I used to have a habit of masturbation (I have seen you mention in your responses to others that masturbation does not cause erectile dysfunction), doing it about 3 to 4 times a week, and everything was normal.
However, after I went to the military, one time when I was on leave and staying at a friend's house, I suddenly felt the urge to masturbate.
After I finished, I noticed that the area around the glans of my penis was swollen, almost like it was filled with water (like a small water balloon).
What does this mean? Will there be any effects or long-term consequences? Recently, this happened again a few weeks ago.
During that week-long leave, while staying at a hotel with friends, I suddenly felt the urge to masturbate while getting ready to shower.
I was feeling quite anxious because my friends were chatting outside.
I remember I pressed on my penis, and halfway through, it completely went soft.
Later, I noticed that my penis felt a bit soft even when not erect, unlike before this incident.
Is this related to frequent masturbation? Although I can still achieve an erection, my penis does seem a bit curved, but the time to ejaculation is not long.
Could this be a cumulative effect of the situations mentioned? Is it possible that I might not be able to function normally anymore, and is there any treatment? Lastly, after using the restroom today, I felt like I hadn’t fully emptied my bladder and ended up expelling a small amount of fluid that resembled semen.
What could be causing this? Am I at risk of infertility? Are my issues serious? What should I do?

Dr. Xu Weikai reply Urology

Hello DIY, I believe it should not affect sexual function.
Localized edema can occur under certain circumstances.
Most individuals have some degree of curvature in their penis; it is rarely completely straight.
As for fertility issues, it seems you might be overly anxious.
This is a response from Dr.
Hsu Wei-Kai, Director of Urology at Taitung Hospital, Department of Health, Executive Yuan.

Hello! Thank you for reaching out with your concerns. It’s important to address issues related to sexual health and function, as they can significantly impact your overall well-being. Based on your description, it seems you are experiencing a few interconnected issues that may be causing you anxiety. Let’s break down your concerns and explore potential solutions.

1. Swelling after Masturbation: The swelling you described around the glans (the head of the penis) could be due to a few factors. It might be a result of excessive friction during masturbation, which can lead to temporary edema (swelling) in the area. This is generally not a cause for concern if it resolves on its own. However, if the swelling persists or is accompanied by pain, discoloration, or other unusual symptoms, it would be wise to consult a healthcare professional for a thorough evaluation.

2. Erectile Function: You mentioned experiencing difficulty maintaining an erection in certain situations, particularly when feeling nervous or anxious. This is quite common and can be attributed to psychological factors such as anxiety, stress, or performance pressure. It’s important to understand that occasional erectile difficulties are normal and can happen to anyone. However, if this becomes a frequent issue, it may be beneficial to explore relaxation techniques, such as mindfulness or breathing exercises, to help manage anxiety during intimate moments.

3. Curvature of the Penis: A slight curvature of the penis can be normal and is often not a cause for concern unless it causes pain or interferes with sexual function. If you notice a significant change in curvature or if it becomes painful, it would be advisable to seek medical advice, as it could indicate a condition like Peyronie's disease.

4. Post-Micturition Dribble: The sensation of not fully emptying your bladder and the presence of a small amount of fluid resembling semen after urination could be related to several factors, including pelvic floor muscle tension or incomplete bladder emptying. This is generally not a serious issue, but if it persists, it would be beneficial to consult a urologist who can assess your symptoms more thoroughly.

5. Concerns About Fertility: It’s understandable to worry about your fertility, especially when experiencing changes in sexual function. However, many factors can influence fertility, and issues like those you described do not necessarily mean you will be unable to conceive in the future. If you have concerns about your fertility, a consultation with a healthcare provider specializing in male reproductive health can provide clarity and guidance.

6. Seeking Help: Given that these issues have been ongoing for 2-3 years, it may be beneficial to seek professional help. A urologist can provide a comprehensive evaluation and address any physical concerns, while a mental health professional can help you work through any anxiety or stress related to sexual performance. Therapy can be particularly effective in addressing performance anxiety and improving your overall sexual health.

In conclusion, while the symptoms you are experiencing may be concerning, many of them can be addressed with appropriate medical guidance and support. It’s important to prioritize your health and seek help when needed. Remember, you are not alone in this, and many men experience similar issues. Taking the first step to consult a healthcare professional can lead to effective solutions and improved well-being.
