Bleeding occurs three days before menstruation?
Dear Doctor,
Over a year ago, my menstrual cycle was normal, but I have been experiencing abnormal bleeding about three days before my expected period (without any discomfort).
Is this normal? A doctor previously mentioned that I have endometriosis (thickening) with a 2 cm cyst that requires a biopsy and hospitalization.
What is a biopsy? Is it possible for me to become pregnant? Thank you very much! A worried mother.

Dr. Xu Junzheng reply Obstetrics and Gynecology
Based on your description, a biopsy may not be necessary.
I recommend seeking a second opinion from another physician.
Thank you!

Abnormal bleeding before menstruation, often referred to as premenstrual spotting or breakthrough bleeding, can be concerning for many women. Understanding the potential causes and implications of this condition is crucial for addressing any underlying health issues and ensuring reproductive health.
Firstly, it is essential to recognize that menstrual cycles can vary significantly among women, and even within an individual’s cycle over time. Factors such as hormonal fluctuations, stress, changes in weight, and lifestyle can all influence menstrual regularity and bleeding patterns. However, consistent abnormal bleeding, especially if it occurs frequently or is accompanied by other symptoms, should be evaluated by a healthcare professional.
In your case, the abnormal bleeding occurring a few days before your expected menstrual period, without accompanying discomfort, could be attributed to several factors. One common cause is hormonal imbalances, which can lead to irregular shedding of the uterine lining. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or even stress can contribute to these imbalances.
You mentioned that a physician suggested the possibility of endometriosis or a thickened endometrium with a 2 cm cyst that requires a biopsy. Endometriosis is a condition where tissue similar to the lining of the uterus grows outside the uterus, often leading to pain and abnormal bleeding. A thickened endometrium can also indicate other issues, such as hyperplasia, which is an overgrowth of the endometrial lining that can sometimes lead to abnormal bleeding.
A biopsy is a procedure where a small sample of tissue is taken from the endometrium (the lining of the uterus) to be examined for any abnormalities, such as cancerous cells or other conditions. This procedure is typically performed in a clinical setting and may require a short hospital stay, depending on the method used. The results of the biopsy can provide valuable information about your uterine health and guide further treatment options.
Regarding your concern about fertility, having a cyst or endometriosis does not automatically mean that you cannot conceive. Many women with these conditions can and do become pregnant. However, the presence of endometriosis can sometimes complicate conception, depending on its severity and location. If you are planning to conceive, it is essential to discuss your concerns with your healthcare provider, who can offer tailored advice and potential treatment options to enhance your fertility.
In summary, while occasional abnormal bleeding can occur, persistent or concerning symptoms warrant further investigation. A thorough evaluation, including imaging studies and possibly a biopsy, can help determine the underlying cause of your symptoms. It is crucial to maintain open communication with your healthcare provider, who can guide you through the diagnostic process and discuss potential treatment options to manage your symptoms and support your reproductive health. Remember, you are not alone in this, and seeking help is a positive step toward understanding and addressing your health concerns.
