Menstrual period has not arrived yet, but there is bleeding?
Hello doctor, my menstrual period usually arrives around the 6th of each month, and this February it came from the 6th to the 12th.
However, today is only February 21st, and yesterday when I went to the bathroom, I noticed some dark red blood, which was not much and felt more like a watery discharge with dry blood stains.
Today, I also noticed a small amount of blood while taking a shower, and I have a feeling of heaviness in my abdomen.
I would like to ask what could be the possible situation?
Zhou, 20~29 year old female. Ask Date: 2016/02/21
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the causes of abnormal bleeding outside of the menstrual period in women may include hormonal imbalances, emotional factors, diet, age, and others.
However, it is crucial to rule out conditions such as uterine tumors, endometrial hyperplasia, cervical lesions, and vaginal lesions.
If the bleeding persists, it is advisable to consult a physician for further evaluation.
Thank you! Wishing you safety and health, Dr.
Zhang.

Unexpected bleeding before menstruation can be concerning and may arise from various causes. Given your description of the bleeding being dark red and not resembling your typical menstrual flow, along with the sensation of abdominal discomfort, several potential explanations could be considered.
1. Hormonal Imbalances: One of the most common reasons for unexpected bleeding is hormonal fluctuations. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or changes in birth control methods can lead to irregular bleeding. Hormonal imbalances can cause the endometrial lining to shed prematurely, resulting in spotting or light bleeding.
2. Ovulation Spotting: Some women experience light bleeding or spotting during ovulation, which occurs approximately two weeks before the expected menstrual period. This is typically light and may be accompanied by mild cramping or discomfort.
3. Implantation Bleeding: If there is a possibility of pregnancy, implantation bleeding could occur when a fertilized egg attaches to the uterine lining. This bleeding is usually light and can be mistaken for an early period.
4. Uterine Fibroids or Polyps: Non-cancerous growths in the uterus, such as fibroids or polyps, can cause abnormal bleeding. They may lead to heavier periods or spotting between periods, and they can also cause discomfort or pressure in the abdomen.
5. Infections: Pelvic inflammatory disease (PID) or sexually transmitted infections (STIs) can cause abnormal bleeding and abdominal pain. These conditions often come with other symptoms, such as unusual discharge, fever, or pain during intercourse.
6. Endometriosis: This condition occurs when tissue similar to the lining inside the uterus grows outside of it, leading to painful periods and irregular bleeding. Women with endometriosis may experience significant abdominal pain and discomfort.
7. Other Medical Conditions: Conditions such as thyroid disorders, clotting disorders, or even certain cancers can lead to abnormal bleeding. If the bleeding is persistent or accompanied by severe pain, it is essential to seek medical attention.
Given your symptoms, it is advisable to consult a healthcare provider for a thorough evaluation. They may recommend a pelvic exam, ultrasound, or blood tests to determine the underlying cause of the bleeding. It is crucial to monitor any additional symptoms, such as increased pain, changes in bleeding patterns, or any new symptoms, as these can provide valuable information for diagnosis.
In summary, while unexpected bleeding can be alarming, it is often due to benign causes. However, it is essential to address any concerns with a healthcare professional to rule out more serious conditions and receive appropriate care. Early intervention can help manage any underlying issues effectively and provide peace of mind.
