What should I do if I develop myopia again after having undergone laser eye surgery?

Hello Dr.
Hu: Two years ago, I underwent laser vision correction surgery, and my preoperative prescription was approximately -6.50 diopters.
Postoperatively, my vision improved to 20/25 to 20/20.
However, during a recent eye examination, my vision was only 20/25 to 20/32, which has made me very concerned about the following:
1.
Have I become nearsighted again? If I have developed nearsightedness again, can I undergo another laser surgery? What are the associated risks?
2.
If I have become nearsighted again, can I still wear contact lenses?
3.
How can I prevent further nearsightedness? Are there any dietary supplements that can help?

Dear Miss Xiao Junjun,
Laser vision correction surgery involves reshaping the cornea to alter its refractive power, thereby replacing the need for glasses.
Myopia primarily results from the elongation of the eyeball, which is defined as the distance from the front to the back of the eye.
If the axial length of the eye continues to increase, myopia will also progress.
If you are concerned about whether your myopia has worsened, it is essential to consult an ophthalmologist for an examination.
The key factor in deciding whether you need to wear glasses or undergo surgery is not merely your visual acuity of 0.8 or 1.2, but rather your satisfaction with your current vision.
If you are not satisfied, further evaluation of the capabilities of current surgical technologies and the potential risks and benefits of the procedure should be discussed with your physician.

As for myopia prevention, current research subjects are limited to preschool children, and there are no studies on prevention in adults.
Eye care includes maintaining a regular routine and adequate sleep, taking breaks every 30 minutes for 5 minutes, and having a balanced diet without the need for additional supplements.
Thank you for your inquiry, and I wish you good health.
Sincerely,
Dr.

After undergoing laser eye surgery, particularly LASIK, it's not uncommon for patients to experience changes in their vision over time. Your concerns about experiencing myopia again after achieving good vision post-surgery are valid and warrant careful consideration. Let's address your questions one by one.

1. Will I become myopic again? Can I undergo another laser surgery? What are the risks?
Yes, it is possible to experience a regression in vision after laser surgery, which can lead to myopia returning. This regression can occur for various reasons, including natural changes in the eye with age, the healing process, or the original degree of myopia being significant. If you find that your vision has decreased to 1.0 or 0.8, it may indicate that you are experiencing some degree of myopia again.

Regarding the possibility of undergoing another laser procedure, it depends on several factors, including the thickness of your cornea, the degree of regression, and the overall health of your eyes. A thorough evaluation by your ophthalmologist is essential to determine if you are a suitable candidate for a second procedure, such as LASIK enhancement or another type of laser surgery like PRK. The risks associated with a second surgery can include complications similar to those experienced during the first surgery, such as dry eyes, glare, halos, or even further regression. Therefore, it's crucial to discuss these risks with your eye care provider.

2. Can I wear contact lenses if I become myopic again?
Yes, if you experience myopia again, you can typically wear contact lenses, provided your eyes are healthy and free from any complications from the previous surgery. However, it’s essential to consult with your eye doctor before resuming contact lens wear, as they will need to assess your corneal health and ensure that your eyes can tolerate lenses without any issues. If you have developed dry eyes or other complications post-surgery, your doctor may recommend specific types of lenses or alternative vision correction methods.

3. How can I prevent myopia from returning? Are there supplements I can take?
Preventing the return of myopia involves a combination of lifestyle changes and regular eye care. Here are some strategies that may help:
- Regular Eye Exams: Schedule regular check-ups with your eye doctor to monitor your vision and eye health. Early detection of changes can help manage them effectively.

- Limit Screen Time: Reduce the amount of time spent on screens (computers, tablets, smartphones) and take regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds.

- Outdoor Activities: Engage in outdoor activities, as studies suggest that spending time outside can help reduce the risk of developing myopia.

- Proper Lighting: Ensure that your reading and working areas are well-lit to reduce eye strain.

- Diet and Supplements: While there is no specific supplement proven to prevent myopia, a balanced diet rich in vitamins A, C, and E, as well as omega-3 fatty acids, can support overall eye health. Foods like leafy greens, fish, nuts, and fruits are beneficial. Some people consider taking supplements like lutein and zeaxanthin, which are thought to support eye health, but consult your doctor before starting any new supplement regimen.

In conclusion, while experiencing myopia again after laser surgery can be concerning, there are options available for correction and management. Regular communication with your eye care provider is crucial to navigate these changes effectively. They can provide personalized recommendations based on your specific situation and help you maintain optimal eye health moving forward.
