Menstruation after abortion?
Hello Doctor: I underwent an abortion on February 17 of this year (and was inadvertently exposed to X-rays).
After the procedure, my first menstrual period was very heavy, but the second and third periods were noticeably lighter and shorter in duration compared to my menstrual cycles prior to the surgery.
Is there any issue with this? What kind of examinations should I undergo at the hospital? I am currently planning to conceive, so I am concerned about potential infertility issues! I have been tracking my basal body temperature and have noted a drop in temperature during my ovulation period; does this indicate that I am ovulating? Thank you for your response.

Dr. Zhong Wenzhen reply Obstetrics and Gynecology
Hello Ms.
Nini: The amount of menstrual flow and the duration of menstruation are not necessarily related to pregnancy, so please do not worry.
If your basal body temperature records indicate ovulation, please rest assured and wait for the arrival of pregnancy.

More Info
After an abortion, it is common for women to experience changes in their menstrual cycle. These changes can include variations in the volume and duration of menstrual bleeding, which may be concerning for some women, especially if they are planning to conceive again.
In your case, you mentioned that your first menstrual period after the abortion was heavier than usual, while the subsequent periods were lighter and shorter than what you experienced before the procedure. This pattern can occur for several reasons.
Firstly, after an abortion, the body undergoes hormonal adjustments as it returns to its pre-pregnancy state. The first menstrual cycle post-abortion can often be heavier due to the shedding of the uterine lining that was built up during pregnancy. As your body stabilizes, subsequent cycles may normalize, which can sometimes result in lighter periods. However, if the changes are significant or if you experience any other symptoms such as severe pain, irregular bleeding, or signs of infection (like fever or foul-smelling discharge), it is essential to consult with your healthcare provider.
Regarding your concern about fertility, it is reassuring to hear that you are tracking your basal body temperature (BBT) and have noticed a drop in temperature during your ovulation phase. A drop in BBT typically indicates ovulation, which is a positive sign that your body is functioning normally in terms of reproductive health. However, it is important to remember that while tracking BBT can provide insights into your ovulation patterns, it does not guarantee that ovulation is occurring every cycle or that conception will happen immediately.
If you are concerned about potential fertility issues or if your menstrual changes continue, it would be wise to schedule an appointment with your healthcare provider. They may recommend several evaluations, including:
1. Pelvic Examination: To check for any physical abnormalities or signs of infection.
2. Ultrasound: To visualize the uterus and ovaries, ensuring there are no complications from the abortion, such as retained tissue or structural issues.
3. Hormonal Testing: To assess your hormone levels, which can provide insight into your menstrual irregularities and overall reproductive health.
4. Hysterosalpingography (HSG): If there are concerns about the patency of your fallopian tubes, this test can help determine if there are any blockages that could affect fertility.
In summary, while changes in your menstrual cycle after an abortion can be normal, it is essential to monitor your symptoms and consult with your healthcare provider if you have any concerns. They can provide personalized advice and ensure that your reproductive health is on track as you plan for future pregnancies. Remember that it may take some time for your body to fully adjust after an abortion, and being patient with this process is crucial.
