Foreskin issues
Hello, doctor.
I have white discharge between the glans and the foreskin of my penis.
What is that? It also has a foul odor.
The most frustrating part is that the area is very sensitive, so I am afraid to use my hands to clean it; I can only use water.
However, washing with water doesn't seem to remove it completely.
What should I do?
Awei, 10~19 year old female. Ask Date: 2006/06/02
Dr. Xu Weikai reply Urology
Awei: Hello, that is smegma, which is a combination of shed skin cells and surrounding dirt.
It is generally recommended to wash it with plain water; using soap or similar products is not advised.
As for whether to use your hands or cleaning tools like towels, that depends on personal preference, as long as it is completely removed.
Note: It is generally believed that smegma may irritate the vagina and is associated with cervical cancer in women.

More Info
When it comes to issues related to the foreskin and the area between the glans (the head of the penis) and the foreskin, the presence of a white substance that has an unpleasant odor can be concerning. This substance is often referred to as smegma, which is a combination of dead skin cells, oils, and moisture that can accumulate in the folds of the skin. While smegma is a normal bodily secretion, excessive buildup can lead to hygiene issues and may cause discomfort or odor.
Understanding Smegma
Smegma is more common in uncircumcised males, as the foreskin can trap moisture and debris. It is important to maintain proper hygiene in this area to prevent the accumulation of smegma, which can lead to irritation, inflammation, or infections. The odor you are experiencing is likely due to the bacteria that thrive in the warm, moist environment created by smegma.
Hygiene Practices
1. Gentle Cleaning: While you mentioned that the area is sensitive and you are hesitant to use your hands, it is crucial to clean the area regularly. You can use a mild soap and warm water to gently clean the area. If you find it uncomfortable to use your hands, consider using a soft washcloth or a gentle sponge.
2. Warm Water Soaks: If direct cleaning is uncomfortable, you might try soaking in a warm bath. This can help loosen any smegma and make it easier to wash away without direct scrubbing.
3. Avoid Harsh Products: Be cautious about using strong soaps or fragrances, as these can irritate the sensitive skin of the penis. Stick to mild, unscented soaps.
4. Drying: After washing, ensure that the area is dried properly. Moisture can promote bacterial growth, so keeping the area dry is essential.
5. Regular Hygiene: Make it a habit to clean the area daily, especially after sweating or physical activity. This will help prevent the buildup of smegma.
When to Seek Medical Advice
If you notice persistent odor, discomfort, or any unusual changes in the skin, such as redness, swelling, or discharge, it is advisable to consult a healthcare professional. Conditions such as balanitis (inflammation of the glans), infections, or other dermatological issues may require medical treatment.
Additional Considerations
- Foreskin Retraction: If you are able to retract your foreskin comfortably, do so during cleaning to ensure that you can clean the glans properly. If you experience pain or difficulty retracting the foreskin, this may indicate a condition known as phimosis, which can require medical evaluation.
- Avoiding Irritants: Be mindful of any products that come into contact with the genital area, including laundry detergents, fabric softeners, and personal care products, as these can sometimes cause irritation.
- Consulting a Specialist: If you continue to experience issues despite following hygiene practices, consider consulting a urologist or a dermatologist who specializes in genital health. They can provide tailored advice and treatment options.
Conclusion
Maintaining proper hygiene is key to managing smegma and preventing associated odors or discomfort. Regular cleaning with mild soap and water, along with being mindful of any changes in the area, can help you manage this issue effectively. If problems persist, do not hesitate to seek professional medical advice for further evaluation and treatment.
