There are many sebaceous cysts on the face?
Hello doctor, I have developed dozens of very small sebaceous cysts on my cheeks and chin.
I consulted a doctor who said they are too small to be surgically removed and that there are quite a few of them.
However, the medication prescribed by the doctor hasn't shown much effect.
The first application caused a little peeling, but after that, there have been no signs of peeling at all.
I feel like my skin might be immune to the medication.
Are there any other options? Would dermabrasion be effective?

Hello: It may be considered to use either a surgical incision or an open procedure, depending on the specific circumstances.

Dealing with multiple sebaceous cysts on the face can be a frustrating experience, especially when traditional treatments seem ineffective. Sebaceous cysts, also known as epidermoid cysts, are common benign growths that can occur on the skin. They are typically filled with keratin and can appear as small, round bumps under the skin. While they are generally harmless, their presence can be bothersome, particularly when they are numerous or located on visible areas of the face.
Treatment Options for Sebaceous Cysts
1. Observation: If the cysts are small and asymptomatic, one option is simply to monitor them. Many sebaceous cysts do not require treatment unless they become inflamed, painful, or infected.
2. Topical Treatments: While you mentioned that the topical medication prescribed by your doctor did not yield significant results, it is worth noting that some topical treatments, such as retinoids, can help in reducing the size of cysts over time. However, they may not eliminate them entirely.
3. Incision and Drainage: For cysts that are inflamed or causing discomfort, a healthcare provider may recommend an incision and drainage procedure. This involves making a small cut in the cyst to allow the contents to escape. However, this method does not remove the cyst wall, which means the cyst can recur.
4. Surgical Excision: If the cysts are bothersome and you desire a more permanent solution, surgical excision is the most effective treatment. This procedure involves removing the entire cyst, including its wall, which minimizes the chance of recurrence. However, as you noted, if the cysts are too small, some doctors may hesitate to perform this procedure.
5. Laser Therapy: Laser treatments can be effective in reducing the appearance of cysts and may help in cases where traditional methods have failed. Laser therapy can also improve skin texture and reduce scarring.
6. Chemical Peels and Microdermabrasion: These treatments can help exfoliate the skin and may reduce the appearance of cysts over time. However, they are not guaranteed to eliminate cysts and should be approached with caution, especially if your skin is sensitive.
7. Lifestyle Modifications: Maintaining a good skincare routine, including gentle cleansing and moisturizing, can help manage the condition. Avoiding heavy makeup and products that clog pores may also be beneficial.
Considerations
- Skin Sensitivity: Given your history of sensitive skin and reactions to certain products, it is crucial to consult with a dermatologist before trying new treatments. They can recommend products that are less likely to irritate your skin.
- Immune Response: If you feel that your skin is becoming resistant to topical treatments, it may be worth discussing alternative medications or therapies with your dermatologist. They may suggest a different class of topical agents or systemic treatments if appropriate.
- Follow-Up: Regular follow-ups with your dermatologist are essential to monitor the condition of your skin and adjust treatment plans as necessary.
In conclusion, while dealing with multiple sebaceous cysts can be challenging, there are various treatment options available. It is essential to work closely with a dermatologist to find the most suitable approach for your specific situation. They can provide personalized recommendations based on the characteristics of your cysts and your overall skin health.
