There are two or three white areas at the site of the circumcision sutures?
Hello Dr.
Hsu,
I would like to ask you about my situation.
I had a circumcision surgery last Saturday, and today is the fifth day post-operation.
The pain has decreased, and I have been cleaning the wound twice daily with saline solution and ointment.
However, I noticed that there are two or three white spots at the sutured area, which makes me a bit worried.
Could you please let me know if this is a sign of infection? Thank you!
Xiao Liang, 20~29 year old female. Ask Date: 2006/08/17
Dr. Xu Weikai reply Urology
Xiao Liang: Hello, in general, it is not recommended to use saline solution for wound cleaning, as it can still cause tissue irritation.
It is sufficient to apply ointment for regular care.
As for the phenomenon of the sutured area turning white, it may be due to the tension of the sutures causing localized tissue ischemia.
It will gradually recover over time.
This is a response from Dr.
Hsu Wei-Kai, Director of the Urology Department at Taitung Hospital, Department of Health, Executive Yuan.

After undergoing a circumcision, it is common to have concerns about the healing process, especially when noticing changes such as white spots at the surgical site. It’s important to understand that the appearance of white spots or areas at the incision site can be due to several factors, and not all of them indicate an infection.
Firstly, the white spots you are observing could be a normal part of the healing process. After surgery, the body goes through various stages of healing, and it is not uncommon for the surgical site to appear white or have a whitish discharge as new tissue forms and the area heals. This can be due to the presence of fibrin, a protein involved in the healing process, which can appear as a whitish film over the incision.
However, it is also essential to be vigilant about signs of infection. Symptoms of infection may include increased redness, swelling, warmth around the incision site, pus or foul-smelling discharge, and fever. If the white spots are accompanied by any of these symptoms, it would be prudent to consult your healthcare provider for further evaluation.
In your case, since you are already cleaning the area with saline and applying ointment as directed, you are taking appropriate steps to care for the surgical site. It is crucial to continue following your doctor's post-operative care instructions. If the white spots do not change, worsen, or if you develop any new symptoms, it would be wise to return to your healthcare provider for a check-up.
Additionally, it is important to avoid any activities that could irritate the area, such as vigorous exercise or sexual activity, until you have fully healed and received clearance from your doctor. Keeping the area clean and dry is vital for preventing infection.
In summary, while the white spots you are seeing could be a normal part of the healing process, it is essential to monitor the site for any signs of infection. If you have any doubts or if the situation changes, do not hesitate to reach out to your healthcare provider for reassurance and guidance. Your health and peace of mind are paramount, and it is always better to err on the side of caution when it comes to post-operative care.
