Post-circumcision issue: There are white spots near the sutures?
Hello, Dr.
Du.
I have diabetes and have been experiencing recurrent balanitis.
Recently, my doctor mentioned that I have warts at the tip of my foreskin due to phimosis.
After discussion, I underwent a circumcision, and the wart lesions were also excised.
Today is the eighth day post-surgery, and I noticed white spots near two of the sutures.
Is this a recurrence of the warts?

Post-circumcision concerns, especially regarding the appearance of white spots near sutures, can be quite common and may cause anxiety for patients. In your case, having a history of diabetes and recurrent balanitis (inflammation of the glans) can complicate the healing process after circumcision.
Firstly, it is essential to understand that after circumcision, the healing process can lead to various changes in the appearance of the skin around the surgical site. The white spots you are observing could be due to several reasons:
1. Normal Healing Process: After surgery, it is common for the area to develop a whitish appearance due to the formation of fibrin, which is a protein involved in blood clotting and wound healing. This can appear as a white or yellowish film over the sutures and is typically not a cause for concern.
2. Suture Material: If absorbable sutures were used, they may also cause some localized irritation or inflammation, leading to a whitish appearance as they dissolve. This is generally a normal part of the healing process.
3. Infection: While less common, it is important to consider the possibility of an infection. Signs of infection may include increased redness, swelling, warmth, pain, or discharge that is yellow or green. If you notice any of these symptoms, it is crucial to contact your healthcare provider.
4. Recurrence of Condyloma (Warts): Given your history of condyloma (commonly referred to as "warts"), it is understandable to be concerned about recurrence. However, the appearance of white spots does not automatically indicate that the condyloma has returned. A healthcare professional would need to evaluate these spots to determine their nature.
5. Other Dermatological Conditions: There are other benign skin conditions that could cause white spots, such as leukoplakia or lichen sclerosus, which may require further evaluation.
Given your diabetes, it is important to monitor the healing process closely. Diabetes can impair wound healing and increase the risk of infections, so maintaining good blood sugar control is essential during recovery.
In conclusion, while the white spots near your sutures could be part of the normal healing process, it is advisable to consult your healthcare provider for a thorough examination. They can provide a definitive diagnosis and, if necessary, recommend appropriate treatment. Regular follow-ups are crucial, especially considering your medical history, to ensure that any potential complications are addressed promptly.
