Green laser

Hello Doctor: My family lives in Changhua.
My father has been experiencing urinary difficulties for the past few years, and an ultrasound revealed symptoms of benign prostatic hyperplasia (BPH).
I have heard about the latest technology, green light laser therapy, for vaporizing and removing excess prostate tissue.
Are there any potential side effects? I understand that some patients who undergo prostate removal surgery may experience erectile dysfunction; will this also occur with green light laser therapy? My father is quite concerned about this.
Is this a temporary condition? Can it be treated or prevented? Additionally, my father has heart issues; is this procedure suitable for him? If you don’t mind, could you recommend a hospital in Taiwan that has specialized equipment and doctors for this surgery? Thank you very much for your response!

Daughter Worried About Her Father, 20~29 year old female. Ask Date: 2006/08/20

Dr. Lai Ronghao reply Urology

Hello: The cost of green light laser surgery is approximately 150,000 TWD.
It is claimed to have fewer postoperative complications, less erectile dysfunction, and less urinary incontinence.
Our hospital does not perform this surgery, but there are several hospitals in Taipei that do.
Please feel free to inquire at major hospitals in Taipei.
Thank you!

Green light laser treatment, also known as photoselective vaporization of the prostate (PVP), is a minimally invasive procedure used to treat benign prostatic hyperplasia (BPH), or enlarged prostate. This technique utilizes a specific wavelength of laser light to vaporize excess prostate tissue, thereby relieving urinary symptoms associated with BPH.

Benefits of Green Light Laser Treatment
1. Minimally Invasive: Unlike traditional surgical methods, green light laser treatment is less invasive, which often results in a shorter recovery time and less postoperative discomfort.

2. Effective Symptom Relief: Many patients experience significant improvement in urinary flow and a reduction in symptoms such as frequent urination, urgency, and nocturia (the need to urinate at night).

3. Reduced Risk of Bleeding: The laser's precision minimizes damage to surrounding tissues and blood vessels, leading to a lower risk of bleeding compared to traditional surgical methods.

4. Shorter Hospital Stay: Patients often go home the same day or the next day after the procedure, which is a significant advantage for those who prefer to avoid lengthy hospital stays.

5. Lower Risk of Complications: The procedure has a lower risk of complications such as infection and urinary incontinence compared to more invasive surgeries.

Risks and Potential Side Effects
While green light laser treatment is generally safe, there are potential risks and side effects that patients should be aware of:
1. Transient Urinary Symptoms: After the procedure, some patients may experience temporary urinary symptoms, such as urgency or frequency, which usually resolve within a few weeks.

2. Retrograde Ejaculation: One of the more common side effects of prostate surgery, including laser treatments, is retrograde ejaculation, where semen enters the bladder instead of exiting through the penis during ejaculation. This is not harmful but can be concerning for some men.

3. Erectile Dysfunction: Although the risk of erectile dysfunction is lower with laser treatments compared to traditional surgery, it is still a possibility. The incidence of this side effect varies, and many men find that any erectile issues improve over time.

4. Urinary Incontinence: While rare, some patients may experience urinary incontinence after the procedure. Most cases are mild and improve with time.

Addressing Concerns About Erectile Dysfunction
Your father's concern about erectile dysfunction (ED) is valid, as it can be a distressing side effect of prostate surgery. However, studies indicate that the risk of ED following green light laser treatment is relatively low compared to other surgical options. If ED does occur, it is often temporary and can improve with time. There are also treatments available, such as oral medications (e.g., PDE5 inhibitors like Viagra or Cialis), vacuum erection devices, or penile injections that can help manage this condition.

Considerations for Patients with Heart Conditions
Given your father's heart condition, it is crucial to consult with his healthcare provider before proceeding with any surgical intervention. The healthcare provider will assess his overall health, the severity of his BPH symptoms, and any potential risks associated with the procedure. In many cases, patients with heart conditions can safely undergo green light laser treatment, but this decision should be made on an individual basis.

Finding a Qualified Provider
In Taiwan, many hospitals and urology clinics offer green light laser treatment for BPH. It is advisable to seek out a facility with a strong reputation in urology and experienced surgeons who specialize in minimally invasive procedures. You may want to consider hospitals affiliated with universities or those known for their advanced urological care.
In conclusion, green light laser treatment for an enlarged prostate offers numerous benefits, including effective symptom relief and a lower risk of complications. While there are potential risks, many patients find the advantages outweigh the drawbacks. It is essential for your father to discuss his specific health concerns with his doctor to determine the best course of action.
