Eyelid surgery (Blepharoplasty)
Dear Doctor: About three weeks ago, I underwent eyelid surgery (external approach) to remove bags under my eyes.
During the surgery, I experienced minimal pain in my right eye, but severe pain in my left eye.
Now, more than a week after the stitches were removed, I feel that my right eye has recovered well, but the outer corner of my left eye appears slightly everted, and the lower eyelid does not move with my facial expressions; it remains fixed in the same position, which looks very strange.
During my follow-up appointment, the doctor mentioned that it is not true eyelid eversion, but rather a result of significant scar contraction causing the eyelid to be pulled down.
He advised that with more massage over time, it should eventually recover.
Since I am getting married at the end of the month, I am really worried that my lower eyelid will remain like this forever.
Is what I am experiencing true? Can significant contraction really cause this? Will it really get better? Thank you!

Currently, the only option is to increase massage therapy.
Your condition may improve in about 2-3 months; however, undergoing surgery 1-2 months before the wedding is somewhat risky.

After undergoing eyelid surgery, particularly for procedures like blepharoplasty (eyelid surgery), it is common for patients to have concerns about healing, scarring, and the overall appearance of their eyes post-surgery. Your experience, especially with the differences between your right and left eyes, is not unusual, and I would like to address your concerns based on medical knowledge and typical recovery patterns.
Firstly, it is important to understand that the healing process after eyelid surgery can vary significantly from person to person. Factors such as individual skin type, the extent of the surgery, and how well one follows post-operative care instructions can all influence recovery. In your case, the left eye's severe pain during the procedure may indicate that it was more affected or that the surgical technique used was more intensive on that side. This could potentially lead to a more pronounced inflammatory response or scarring.
The phenomenon you are experiencing with the left eye, where the lower eyelid appears to be fixed and does not move with facial expressions, can indeed be attributed to scar tissue formation. Scar tissue can sometimes contract more than normal skin, leading to a condition known as "contracture." This can create a pulling effect on the eyelid, which may give the appearance of ectropion (outward turning of the eyelid). Your surgeon's recommendation to massage the area is a common approach to help break down scar tissue and encourage more normal movement of the eyelid.
Regarding your concern about the potential for permanent changes, it is essential to remain optimistic. Many patients experience significant improvement in the months following surgery as the scar tissue matures and softens. The first few weeks post-surgery are critical, and while the initial appearance may be concerning, the healing process can lead to a more favorable outcome over time.
However, if the situation does not improve or if you notice any signs of complications, such as increased pain, redness, or discharge, it is crucial to follow up with your surgeon. They may recommend additional treatments, such as corticosteroid injections to reduce scar tissue formation or even further surgical intervention if necessary.
As for your upcoming wedding, it is understandable to feel anxious about your appearance. While it is difficult to predict the exact outcome, many patients find that by the time they reach significant events, their healing has progressed well enough to feel confident. In the meantime, consider discussing your concerns with your surgeon; they may provide additional reassurance or strategies to help you feel more comfortable leading up to your wedding day.
In summary, while your concerns about scarring and eyelid movement are valid, the healing process can lead to improvements over time. Following your surgeon's advice on massage and care, along with regular follow-ups, will be key in achieving the best possible outcome. Remember, patience is essential in the recovery process, and many patients find that their results improve significantly in the months following surgery.
